- Contractor to provide Community-Based Primary Care services to veterans of the Department of Veterans Affairs Medical Center, Butler, PA, for base year plus two (2) One-year renewal option periods. Contractor’s facility to provide services must be physically located in Clarion County, PA. Pricing is on a “per member per month (PMPM)” basis and represents an “all inclusive” reimbursement for routine delivery of primary care. Offerors should quote a monthly capitation rate per patient.
