- Description
- The Health Resources and Services Administration (HRSA) will be soliciting proposals for Spread of the Medical Home Concept. The purpose of this contract is to use a quality improvement process to increase the portion of children and youth with special health care needs that have access to community-based, comprehensive, coordinated care in the context of a medical home. This contract will be conducted through two concurrent and interwoven components: 1) Strengthening State (Title V) capacity to support implementation and spread of the Medical Home and 2) Leading a Medical Home collaborative for primary care practices. The Government anticipates awarding a 15-month contract. This acquisition is being conducted under Full and Open competition. The solicitation will be issued on or about July 16, 2004. It will be available at the Government website at www.fbo.gov under HHS, Health Resources and Services Administration, Posted Dates. It is the offeror's responsibility to monitor the site for amendments to the solicitation. The agency will consider proposals from all responsible sources.
