Description

The VA MEDICAL CENTER, DAYTON OH is soliciting proposals to provide wheelchair van transportation services for the VAMC, Dayton, Ohio. This is a firm fixed price 100 percent small business set-aside. Period of performance is from date of Award thru July 31, 2005 with four option years. Electronic version and hard copy of solicitation will be available on or about June 15, 2004. Electronic solicitation may be obtained FREE at the following URL: http://www.bos.oamm.va.gov./ Offerors should contact Michelle.ranes@mail.va.gov or (540) 842-4699 regarding any problems downloading the solicitation from the website. The Contracting Officer for this solicitation may be contacted at Samuel.Tahmoush@med.va.gov or at (937) 262-2120. Requests for hard copy solicitation must be in writing to the VA Dayton A&MM Contact Point and accompanied by a check for $5.00 made out to the Department of Veterans Affairs. No telephone requests will be accepted. Any amendments issued to this solicitation will also be available at the URL address. Offerors are advised that they are responsible for obtaining amendments. See numbered note 1.
