Description
This is a combined solicitation/synopsis for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a separate written solicitation will not be issued. The solicitation number is 263-04-P(GQ)-0053 and is being issued as a Request for Proposal (RFP). The solicitation/contract will include all applicable provisions and clauses in effect through Federal Acquisition Circular (FAC) #2001-23. The NAICS Code is 485111 and the business size is $6.0. The National Institutes of Health (NIH) in Bethesda, Maryland has a requirement for Fare Media (metro-rail fare-cards, metro-checks, and metro-bus tokens). The NIH intends to contract with the Washington Metropolitan Area Transit Authority (WMATA), 600 Fifth Street, NW, Washington, DC 20001. The NIH believes that WMATA is the only source that can provide this requirement. The WMATA is the sole issuer of the fare media to be obtained on this proposed acquisition. This action is taken under the authority of 41 USC 253(c)(1), FAR 6.302-5. The contract shall be for one year with four twelve-month option periods. Any other firm that believes it can meet the requirement must submit by the response date (15 calendar days from date of this publication) a detail capability statement documenting their ability to provide this service by meeting the requirement stated above. Information provided shall include enough material to perform a proper evaluation. A complete copy of the provision FAR 52.212-1, Instructions to Offeror - Commercial Items and a complete copy of the provision FAR 52.212-3, Offeror Representations and Certifications - Commercial Items is required as part of the offer. The latter provision includes the DUNS Number Addendum. The resulting contract will incorporate the requirements of the clause at FAR 52.212-4, Contract Terms and Conditions ? Commercial Items, and FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders ? Commercial Items. FAR 52.232-19, Availability of Funds for the Next Fiscal Year, FAR 52.216-19, Order Limitation, FAR 52.216-2, Economic Price adjustment-Standard Supplies, FAR 216-21, Requirement, FAR 217-8, Option to Extend Services, FAR 52.217-9, Option to Extend the Term of the Contract, FAR 52.216-18, Ordering, FAR 52.222-26, Equal Opportunity; FAR 52.222-35, Affirmative Action for Special Disabled and Vietnam Era Veterans; FAR 52.222-36 Affirmative Action for Workers with Disabilities; FAR 52.222-37, Employment Reports on Special Disabled Veterans and Veterans of Vietnam Era; and FAR 52.225-3, Buy American Act ? Supplies. Full text copies of the representations and certifications or other cited provisions and clauses may be obtained from http://arnet.gov/far/. A complete and signed ?Offeror Representation and Certifications ? Commercial Items ? with DUNS Number Addendum must accompany offers. The Contractor will be paid following delivery and acceptance of the services/supplies. The proposed contract action is for supplies or services for which the Government intends to solicit and negotiate under the authority of FAR 6.302.
