Description

This is a combined synopsis/solicitation for the acquisition of commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in solicitation. For Contractor?s convenience separately attached is SF-1449, Solicitation, Contract, Order for Commercial Items and incorporated provisions and clauses in effect through FAC-01-23. The applicable NAICS is 334510; size standard is 500 employees. The USDA Forest Service intends to purchase (22) Automatic External Defibrillators (AEDs) Units which includes Medical Oversight; training and travel to five (5) locations and freight. Contractor is to include discounts, warranties, packing and shipping costs and catalog information on AEDs quoting. Contractor shall address all evaluation factors outlined in FAR 52.212.2 and complete specification table. Equipment will be shipped to USDA Forest Service, 11 Campus Blvd., Ste 200 Newtown Square, PA 19073. Written quotes are due at the USDA Forest Service, 11 Campus Blvd., Ste 200 Newtown Square, PA 19073 ATTN: Patricia Pierce by 3:00pm on June 24, 2004. Faxed quotes will be accepted 610-557-4224 or 4200. Contractors are responsible to make sure fax quotes reach Contracting Officer timely.
