Description
The National Hansen?s Disease Programs (NHDP) is a long-term health care facility for patients with Hansen?s Disease. The NHDP leases the 3rd floor of Summit Hospital in Baton Rouge LA. The are 30 patient beds and the average patient population is 20 patients. The requirement of this contract is to supply all labor, food, materials, supplies and transportation for complete meal and nutrition service for the patients assigned to the NHDP ward. There are no facilities within the government?s leased space for food preparation, serving tray preparation, storage (except as indicated below), clean up, food warming, refrigeration (except as indicated below) or disposal. The contractor shall provide complete nutrition service for patients assigned to the hospital ward of the NHDP in Baton Rouge LA. This shall include regular diets, soft diets, pureed diets, High CHO, High Protein diets, diabetic diet, 2 GM sodium diets, bedtime snacks, nourishment, yogurt, juice, and meal supplements/substitutes such as Ensure, Promod, Jevity and others as specified by physicians. This is an estimated contract. Payment shall be based upon actual servings provided. Meals shall be prepared and served in accordance with JCAHO requirements. The contractor?s food preparation facility shall be no further than 10 miles from the NHDP unit. The commuting time shall be no longer than 15 minutes. Food shall be prepared in contractor spaces and delivered to the HD unit. Breakfast: No earlier than 7:30AM and no later than 8:00AM. Lunch: No earlier than 12:00PM (Noon) and no later than 12:30PM. Dinner: No earlier than 4:30PM and no later than 5:00PM. Bedtime Snacks shall be delivered with dinner and marked. Supplements, juice, yogurt and nourishment shall be replenished daily as required. Meals shall include selections identified below. Menus shall be on a rotating basis. Fresh fruit shall be served with every meal (whole fruit ? bananas, apples, oranges, cantaloupe or other melons (these can be cut up) or fruit is season)). Steamed rice shall be served for lunch and dinner daily. Breakfast ? Fruit or juice, hot cereal, eggs, breakfast meat, toast or biscuit, milk, coffee Lunch/Dinner ? Soup, 3 oz. meat serving, starch, vegetable, salad, dessert, fruit, milk, coffee or tea. Bedtime Snack ? Meat sandwich, 8 oz. milk, fresh fruit or juice. Nourishment ? Graham crackers, saltines, Kool-Aid, soft drinks, jello, custard, Gatorade, yogurt, milk, canned soups, soft drinks, ice cream, juice and coffee throughout the day as needed/desired. Alternate Menu Selections ? A light menu selection shall be provided for patients who do not want the regular menu. Light menu selections shall include combinations of soup, salad, sandwiches, hot cereal, etc. The contractor shall provide a menus on a 4-week rotational basis. The quantity and types of meals (regular meals, diabetic, etc) for each day will be provided to the contractor by the nursing staff not later than 5:30AM. However, due to fluctuating patient population, changes may be required throughout the day and on short notice. Orders for meal service shall be available up to 6:30PM to accommodate late patient check in. The contractor shall provide a contingency plan for fulfilling changing needs. The following factors shall be considered when planning meals and serving/eating utensils. The patients on the NHDP ward are a varied ethnic population. Menu choices offering various ethnic selections shall be rotated. These selections shall include beans on the menu at least one per week, fried fish, fried or boiled eggs, tamales and other Mexican/Spanish selections. Some of the patients have various handicaps, including insensitivities in their hands and limited use of hands. Hot beverage containers shall be insulated. Food shall be served in containers to accommodate their handicaps. Menu choices shall consider the various handicaps. The patients are elderly. Menu choices shall consider the needs and desires of elderly patients. Meals shall be served using traditional glass and metal dishes, glasses and eating utensils. Hot beverage containers shall be insulated. The contractor is responsible for providing all food preparation, serving dishes (trays), dinnerware (plates, cups, glasses, bowls, etc) utensils (knives, forks, spoons) and napkins for meal service. All items required for the patient meal (food, plates, cups, bowls, food, utensils, etc.) shall be placed on the patient tray and ready to give to the patient to begin eating. The patient trays shall be placed on rolling service carts and delivered to the NHDP ward on the third floor at the appointed time for the meal being served. After patients have finished eating, the trays (including plates, cups, bowls, utensils and any debris) will be returned to the carts by NHDP staff. The contractor shall pick up service carts with trays not later than 1? hours after the meal has been delivered to the site. There is no disposal area in the ward area. The contractor shall remove all uneaten food and trash from the premises and dispose at the contractor?s facility. Storage space, heating and refrigeration is available for juice and nourishment which is provided to patients throughout the day as needed or desired. A ?par level? of the various items sufficient to provide for patient needs shall be established, initially stocked by the contractor and replenished daily. This contract provides the only source for the nutritional needs of the patients assigned to the ward. The contractor shall provide a contingency plan for providing patient meals in the case of unforeseen events, whether or not they are caused by or within the control of the contractor (such as, but not limited to events such as power outages, accidents, traffic, weather related delays, etc.) The contractor shall comply with all Federal, state and local laws, regulations, ordinances regarding food preparation, serving, storage, disposal etc. Nothing in this contract shall relieve the contractor from compliance with such laws, regulations, statues and ordinances. Contractor shall be or shall be capable of being accredited by the Joint Commission for Accreditation of Health Organizations. The request for proposal will be posted on or about June 24. The North American Industry Classification System (NAICS) code is 722310 and the Small Business Size Standard is $17.5 million. The Contractor must have experience in providing hospital food services. The firm must have gained this experience as a result of being regularly engaged in the business of providing food service in health care/patient care environments (e.g. inpatient and outpatient medical treatment facilities.) Only firms with this experience will be considered for award. The contractor shall be available during normal duty hours and shall be able to meet on the installation with government personnel within 24 hours after notification. All interested parties must be registered in the Central Contractor Register (CCR) in order to receive an award. The solicitation will be issued on or about June 24, 2004.
