- This is a combined synopsis/solicitation for commercial item(s) prepared in accordance with FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the solicitation. Proposals are requested and a w ritten solicitation will not be issued. W91YTZ-04-T-0113 is issued as the Governments intent to award sole source (Note 22) to Electro-Diagnostic Imaging for the Veris Science System (Multifocal Retinography). This system consists of a Microdisplay Stimul ator with fundus camera, interface electronics including articulating arm and grid calibrator; Macintosh G4 Computer System with Data acquisition board Interface unit and cabling; Isolation transformer including DC power; Grass Amplifier; Veris Science sof tware; Auto calibrator; Science Manual; 17 inch Apple display; switchbox; Borian Allen IR electrode-adult with battery pack; 4LED Cobra and battery pack; Macintosh compatible printer; 48 inch Anthro cart with side rack for CPU and mounting for articulating arm; Installation; and training. The solicitation document and incorporated Provisions and Clauses are those in effect through Federal Acquisition Circular (FAC) 2001-23. The SIC Code is 3841; the North American Industry Classification System (NAICS) code is 339112. The North Atlantic Regional Contracting Office intends to issue a firm fixed price contract for one Veris Multifocal Electroretinography system at Walter Reed Army Medical Center (WRAMC), Washington DC 20307. The following Provisions and Clause s are incorporated and shall remain in full force in any resulting purchase order: FAR 52.212-1 Instructions to Offerors- Commercial Items; FAR 52.212-2 Evaluation of Commercial Items. FDA-approval for the clinical use of this product is required. Technic al capability and price are approximately equal; Offeror is to provide with quote a list of 3 references by contract number, point of contact name and phone. The Government reserves the right to make award on the initial proposals received without discussions. The Government will consider all quotes received by 11 June 2004.
