Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being accepted and a written solicitation will not be issued. Solicitation Number FA3089-04-T-0016 is issued as a request for quotation for Paramedic/Ambulance Support Services for Randolph AFB, TX (a copy of the bid schedule, statement of work, and addendum to clauses added as amendments to this notice). Dept of Labor Wage Determination 94-2521, Revision 26, for Bexar County, Texas applies and can be accessed through the Internet at https://www.ceals.usace.army.mil/netahtml/srvc.html. NOTE: Per Dept of Labor All Agency Memorandum Number 197, dated 24 May 2004, the Health and Welfare Benefits Rate effective 1 June 2004 will change from $2.36 to $2.59 per hour. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-22 & 23. This requirement is unrestricted (Full & Open Competition) utilizing the Size Standard in Federal Acquisition Regulation (FAR) Part 19 and NAICS Code 621910; with a size standard of $6.0M. The government anticipates awarding a one-year base contract period with four available 12-month option periods.
