- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in the attachment. The United States Attorney Office-Northern District of West Virginia has a requirement for Grand Jury and Deposition Reporting Services for the period of twelve months. It is anticipated that award will be made for five (5) years. The implementation of each year will be at the Government=s discretion. Offerors must have the capability to establish an office within a four hundred (400) mile radius from WHEELING, CLARKSBURG, ELKINS, AND MARTINSBURG city limits by shortest route available. This notice is prepared in accordance with FAR Part 13, Simplified Acquisition Procedures, NAICS 561492 with size standard $6M. THIS IS A TOTAL SMALL BUSINESS SET-ASIDE PROCUREMENT. QUOTES ARE BEING REQUESTED. The solicitation number is RFQ-EOA87-4-0068. See ATTACHMENT for the following additional information: Instructions for completion of solicitation package, Schedule of Commercial Services and Prices, Statement of Work and Applicable clauses 52.2121, "Instructions to OfferorsCommercial Item", 52.2123 "Offeror Representations and Certifications Commercial Items", 52.2124 "Contract Terms and Conditions Commercial Items" and 52.2125 "Contract Terms and Conditions Required to Implement Statutes or Executive Orders  Commercial Items. Offers or quotations will be evaluated in accordance with FAR 13.1062 "Evaluation of quotations or offers". Quotes are due June 1, 2004 at 2:00pm, Eastern Standard Time.
