Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. This reference number is F7382240820100 and is being issued as a Request for Quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-22 & 23. This acquisition is 100% set-aside for small businesses. The associated NAICS code is 611710 with a size standard of $6,000,000. The basic contract period is 15 June 04 - 30 Sep 04 with four option years. The offeror shall provide as a response to this solicitation: (1) a resume detailing relevant training and experience; (2) a list of at least two recent references with contact information; (3) Monthly price for the basic contract period, option year 1, option year 2, option year 3, and option year 4. All responsible small business sources may submit a quotation, which shall be considered by the agency. STATEMENT OF WORK: (Environment) (a) The Catholic religious education coordinator provides key leadership in the design and implementation of Catholic services and programs. These services are provided for pre-kindergarten to 12th grade children, as well as adults, to include confirmation to assist in building a solid foundation for spirituality and sound character development. (b) The Contactor shall perform primary duties on Charleston AFB, SC 29404 buildings 500 and 245. (Contractor?s Duties and Responsibilities) (a) Contractor shall successfully under go a background check completed by Charleston Air Force Base (CAFB) Office of Special Investigations (OSI). (b) Contractor shall maintain a neat, clean and well-groomed professional appearance and conduct self in a business-like manner. (c) Contractor will have set office hours on Sundays from 10:30am to 1:00pm; all other days contractor will work necessary hours to complete duties within this SOW. The level of effort is approximately equal to 10hrs per week. (d) Contractor shall give the Catholic chaplain or his designee a two-week notice of any extended period of absence (one week or more). The contractor shall provide a substitute to perform services in their absence. This substitute must be of equal or greater capability than the original contractor and approved by the Catholic chaplain or his designee. The contractor shall develop a documented method of reimbursement for any substitutes and is responsible for paying any costs for the substitute. (e) Contractor shall use Microsoft Office files in word processing, excel spreadsheet, and power point software and other technological systems as needed, such as multi-media and sound systems. (f) Contractor shall attend Catholic council meetings and separate meetings as determined by the Catholic chaplain. Routinely provide and submits required reports (Catholic education status reports, inventory of Catholic education supplies, after action report, etc.) to the council meeting members and Catholic chaplain as designated by the Catholic chaplain. (g) Contractor shall properly receive and account for all Catholic religious educational funds to submit to the account technician per CAFB Chapel local instructions. (h) Contractor shall develop a proposed 12-month calendar of events and scheduled classes to be submitted to Catholic chaplain. (i) Contractor shall recruit and adequately train volunteers to staff the Catholic religious education program. (j) Contractor shall maintain and account for all Catholic education supplies. (k) Contractor shall revise and update all Catholic education documents for the Catholic community, which includes, but not limited to: Parent handbook of policies, procedures, and class calendar (approved by Catholic chaplain), sacramental guidelines, and archdiocesan requirements and norms. (l) The contractor shall comply with all Air Force computer and information security requirements. (Government furnished equipment) (a) Computer, computer programs, work area, standard office supplies. END OF STATEMENT OF WORK. The Government will award a contract resulting form this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. The following factors shall be used to evaluate offers: Past Performance and Price. The Government will evaluate offers for award purposes for the total price for the requirement. Past performance and Price, when combined, are approximately equal. Contractors shall include a completed copy of FAR 52.212-3, Offerors Representation and Certifications Commercial Item FAR 52.212-4 Contract Terms and Condition?Commercial Items; FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders?Commercial Item are incorporated by reference, however, for paragraphs (b) and (c) the following clauses apply; FAR 52.217-4 ?Evaluation of Options Exercised at Time of Contract Award; FAR 52.217-5 ?Evaluation of Options; 52.217-8 -Option to Extend Services. The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 60 days. FAR 52.217-9 -Option to Extend the Term of the Contract. (a) The Government may extend the term of this contract by written notice to the Contractor within 15 days; provided, that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option clause. (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 5 years 6 months.
