DESCRIPTION: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation document will not be issued. The Veterans Affairs Medical Center Sheridan, Wyoming intends to enter into a firm fixed price contract to provide Dairy and Dairy products required at the V A Medical Center, Sheridan, WY. Request for Quotations (RFQ) # 666Q-04-2 entitled "Dairy and Dairy Products" is hereby issued for commercial items as an Unrestricted solicitation and is prepared in accordance with the format in Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items, Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, FAR Part 13. This announcement incorporates provisions and clauses in effect through FAR FAC 2001-22 and VAAR 97-10. NAICS Code 311511 applies for this procurement and the small business size standard is 500 employees. Contractor's quotes are herein requested. A response to requirements listed below shall be submitted on contractor's letterhead or formal quotation form to include pricing for all line items in the quotation schedule, base and two option years. Contractor's price quote must contain the name; title and signature of person authorized to submit quotes on behalf of the contractor. The contractor will provide the following services in accordance with requirements of the contract: 1. DELIVERIES OR PERFORMANCE Ordinarily deliveries will be required to be made not less than three (3) days per week, preferably Monday, Wednesday and Friday. If a holiday falls on one of these days, delivery on the holiday will be expected unless prior arrangements have been made with Nutrition and Food Service. Deliveries to the Veterans Canteen Service establishment will be required no more often than deliveries to the Sheridan VA Medical Center. All deliveries must be made after 7:30 a.m. and completion of all deliveries made by 9:00 a.m. All milk will be placed in the refrigerator by the driver. Current stock will be moved to the right side in the refrigerator and delivered milk will be placed inside the refrigerator on the left. 2. MULTIPLE USE CONTRACT The estimated quantities include the estimated requirements for the Veterans Canteen Service (VCS) at this Medical Center. Orders from the VCS against any contract resulting from this proposal will be honored by the Contractor at the contract prices. Arrangements for placing orders and deliveries will be made by the Canteen Officer and shall be separate and distinct from orders, deliveries and billings for supplies furnished the Department of Veterans Affairs Medical Center. 3. PAYMENTS Payment will be made monthly upon receipt of properly prepared invoices for items delivered during the preceding month. Invoices are to be submitted to the Chief, Fiscal Service (04), 1898 Fort Road, VA Medical Center, Sheridan, WY 82801. Invoices for the Canteen deliveries are to be submitted directly to the VA Canteen Service. Contractor agrees to accept payment via Electronic Funds Transfer between the US Treasury and the Contractor?s designated bank account. Contractor agrees to complete SF 3881,and return it within five days of notice of contract award. 4. TERMINATION The contract is effective for one year after date of award with option years if exercised. The contract is subject to availability of funds for the fiscal year, therein, therefore; the Contracting Officer must be contacted for services rendered after 9/30. The contract may be terminated by either party by giving ninety (90) days advance written notice of the effective date of termination. 5. CONTRACT ADMINISTRATION DATA The Contracting Officer as the agent of the Government, with authority to negotiate terms, execute legally binding agreements affecting price, quantity, or quality of performance under this contract and administer contracts on behalf of the Government. The Contractor shall communicate with the Contracting Officer on all matters pertaining to contract administration. In the event the Contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustment will be made in the contract price to cover any increase in costs incurred as a result thereof. 6. CONTRACTING OFFICER?S TECHNICAL REPRESENTATIVE Typically the program manager of Nutrition and Food Service and VA Canteen. He/she is the technical advisor to the Contracting Officer and is responsible for the daily technical oversight of the Contractor?s work and for making recommendations to the Contracting Officer regarding matters of contract administration. The foregoing is not to be construed as authorization to interpret or furnish advice and information to the Contractor relative to the financial or legal aspects of the contract. Enforcement of these segments is vested in and is the responsibility of the Contracting Officer. 7. CERTIFICATIONS In offering under this solicitation, the oferor certifies that their company has received, prior to opening of bids, a pasteurized milk rating of 90% or more for the type of product they propose which will be in accordance with specifications as found in Federal and State regulations. The Contractor will provide certification that plant inspection has taken place within the past three months by either the State or Federal Agencies. Pasteurized Milk Rating: Date of Rating______. AGE OF PRODUCT The finished products shall be delivered a minimum of seven (7) days prior to the date coded on the individual containers. Nutritive breakdown on packaging should reflect specifications. The Offeror shall also provide its Commercial and Government Entity code, DUNS number, and Tax Identification number. Offerors shall submit quotes as soon as possible to James C. As of October 1, 2003, all Contractors, both large and small, are required to register in the Central Contractor Registration (CCR) database before an award.
