- Amendment no. 001 is hereby issued to revised RFQ-04-0511 from Sole Source to soliciting from all sources for Badges for Police Officer at Bureau of Engraving and Printing in Washington D C and Fort Worth TX to purchase two hundreds and forty two (242 ea), Design selected #2C and change the closing date from 05/17/04 to 05/24/04. All vendors shall submit a product samples along with their quotations. The simplified acquisition procedures set forth in FAR 12 and 13 shall be used. This is a combined synopsis/solicitation. This notice constitutes the only solicitation; a written request for quotation will not be issued. This acquisition is 100% set-aside for small business (see numbered note 1). The applicable North American Industry Classification system (NAICS) code 339999, with a small business size standard is 500 employees. The request quotation (RFQ) number is RFQ-04-0511. Delivery shall be made within 30 days after receipt of purchase order. Deliver are F.O.B. Destination to Bureau of Engraving and Printing, D Street between 13th & 14th Street S W., Washington D C 20228. The resulting purchase order shall be firm fixed price. The basis for award shall be price. All interested small business is requested to submit a written quotation, which will be considered by the agency. Quotation may be submitted on a SF1449, SF18 or on company letterhead. Quotations shall include the RFQ number, the proposed unit price, the proposed extended price, any prompt payment discount terms, company name, point of contact, address, phone number, fax number, tax identification number. DUNS numbers, size status (that is small business, disadvantaged business or woman owned business). If this item is available under GSA Schedule, please provide the schedule number. Quotations are due by 2:30 P.M. on May 24, 2004. Quotations may be mailed to the Bureau of Engraving and Printing (BEP), Office of Procurement, Attn: Rosa Lee, 14th and C Street, S.W., Washington, D.C. 20228-001 or faxed to Rosa Lee at 202-874-2200.
