Classification Code: X Set Aside: N/A Contracting Office Address: Executive Officer for United States Attorneys, 600 E Street NW, Suite 2400, Washington, DC 20530. Description: The Executive Office for United States Attorneys (EOUSA) intends to issue a purchase order contract in support of the annual 4 Corners Indian Nation Conference in the Phoenix metro area. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in the solicitation notice (SEE ATTACHED). This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be provided. The solicitation number is RFQ-EOA06-4-0067. The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-21; Schedule of Commercial Services and Prices, Statement of Work and Applicable clauses 52.212?1, "Instructions to Offerors?Commercial Item", 52.212?3 "Offeror Representations and Certifications ?Commercial Items", 52.212?4 "Contract Terms and Conditions? Commercial Items" and 52.212?5 "Contract Terms and Conditions Required to Implement Statutes or Executive Orders ? Commercial Items. The lodging facilities should be within the Phoenix metropolitan area. The North American Industrial Code is (NIAC) 721110, size standard $6.0 million. This service shall be provided in accordance with the terms, conditions of the proposed order/contract and Statement of Work set forth in the solicitation. The evaluation processes will apply as follows: In evaluating quotes, the Federal Government intends to use the following factors: a. Meeting the requirements to contract with the Federal Government Dun & Bradstreet Number (DUNs) (Central Contractor Registration (CCR), Tax Identification Number (TIN). b. Meet the time or period of the event c. Sleeping Room Capacity d. Meeting Room Capacity e. Break out/training room's accessibility/capacity f. Audio visual support g. Safety and Security requirements h. Environmentally Controls i. Price/Government per diem In determining award, the Federal Government intends to use the best value process in selecting a contractor for award. INSTRUCTIONS 1. Fill in the prices on the attached pricing sheet, (Section B, Schedule of Prices). The document will be posted in MS Word so you may submit electronically, and hard copy. In both platforms you must reference the RFQ number as the subject. Please remember to type or print legibly, verify the math and complete the totals. 2. Read and complete Section E (Solicitations and Provisions) This section must be returned with your quote. 3. Please identify your legal company name, address, phone, and fax numbers, a point of contact and an e-mail address if possible. The entire package must be submitted prior to the closing date and time. May 17, 2004 @ 12 noon Eastern Standard Time (EST). Late quotes will not be considered. Additional information 1. The Government may also eliminate a proposed contractor based on the Government's knowledge and/or experience of a vendor's poor performance history. 2. The Government may terminate the contractual agreement at any time if a contractor knowingly violates any policies, provisions or procedures governing the order/contract. 3. Attention: Department of Justice Contractors All current and prospective contractors interested in doing business with the Federal Government must be registered in the Central Contractor Registration (CCR) database. Contractors not registered in CCR will be ineligible to receive future contract awards or receive payments under existing contracts. You will need a Dun & Bradstreet (D&B) number to register. If you do not have a D&B number, contact Dun & Bradstreet, Inc. at (8000 333-0505 or you can obtain a DUNS number via the Internet at http://sbs.dnb.com There is no charge to register in CCR. Registration can be completed on line at www.ccr.gov . If you do not have Internet access, contact the CCR Assistance Center toll free at 1-888-227-2423, they will also provide additional guidance on the registration process.
