Description
This is a combined synopsis/solicitation for commercial items in accordance with the format in Federal Acquisition Regulations (FAR) subpart 12.6, Streamlined Procedures for Evaluation and solicitation for Commercial Items as supplemented with additional information included in this announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. RFQ 12004-0022-4 is issued as a request for quotation. The solicitation document and incorporated provisions are those in effect through Federal Acquisition Circular 2001-23. The North American Industrial Classification System Codes is 337110. The small business standard is $8.5 Million. This solicitation is 100% set-aside for small business. Faith-based and Community-based organizations have the right to submit offers equally with other organization for contracts for which they are eligible. The Government intends to make a single award of a firm fixed price contract. Award will be made to the responsible quoter who submits the most advantageous offer to the Government based on price. GENERAL REQUIREMENTS: The Federal Bureau of Prisons (BOP), United States Penitentiary (USP) McCreary, located at 330 Federal Way, Pine Knot, Kentucky, has a requirement for cabinetry and counter tops to be installed in the Health Services Areas of the USP. Award is estimated to be on or about May 17, 2004. The successful contractor shall delivery and install cabinetry and counter tops within 14 days of contract award. The work shall conform to State and Local codes and cabinet work is to be fabricated in such a manner that no noticeable variances will be visible between existing and new work. Base & Wall construction in Health Service Areas can be 3/4" thick x 1 ?? wide face frames of solid maple. Center post on double door cabinets are to be minimum width of 3". Base cabinet shelves will be half depth and will withstand a minimum of 15 pounds per square foot. Wall cabinet shelves 24", 30", 36", & 42" high cabinets feature adjustable shelving. Base and wall interiors will be constructed of natural maple Aristokraft (or approved equal). Hardware will be installed to meet the specifications and drawings and shall be fully concealed self closing steel hinges. Door construction shall be antique (off white) smooth satin thermofoil, medium density fiberboard, and feature raised panel door. The Pharmacy work area will not require door fronts to be installed. Cabinet doors that require glass are to be constructed using tempered safety glass materials. Cabinet doors that meet in the middle should have a clearance of 1/8" and exposure to base cabinets should be limited. Counter-tops are to be constructed of an antifungal/antibacterial solid surface and Hi-macs will be lowest quality acceptable. The drawers must be center mounted drawers. The contractor will be required to furnish a full one year warranty of all hardware. Additionally the contractor is required to furnish a full three (3) year warranty for cabinet construction. All installations are to be coordinated with the Health Service and facilities staff and are not to slow down nor halt the normal operations of the Institution. Contractors are to have personnel present to receive all shipments unless otherwise approved. SUPPLIES: The contractor shall provide the following supplies, fasteners, hardware and installation of supplies: Design, supply and install 228 linear feet of Aristokraft Nantucket Antique (or approved equal) select cabinetry with hardware. Supply and install 205 linear feet of Hi-macs Desert Sand (or approved equal) solid surface counter tops with 2 under mount stainless steel sinks. The counter tops must be made of non-porous solid surface material. Provide one (1) General Electric (or approved equal) black on black under counter refrigerator. Provide 2 (two) Delta (or approved equal) faucets. Deliver and Install cabinetry in a professional manner and remove cardboard and other packing materials to the nearest dumpster. Installation includes all fasteners and hardware. Plan Views/ Drawings are available upon request. The Government will make award based on technical compliance and price. CONTRACT CLAUSES/ SOLICITATION PROVISIONS: The following FAR clauses/provisions apply to this acquisition: 52.204-6, Data Universal Numbering System; 52.204-7, Central Contract Registration; 52.211-6, Brand Name or Equal 52.212-1, Instructions to Offerors Commercial Items applies to this acquisition, (period of acceptance of offers) is deleted as not applicable; 52.212-4, Contract Terms and Conditions Commercial Items; 52.214-31, Facsimile Bids; 606-354-7022 52.219-6, Notice of Total Small Business Set-Aside; 52.219-8, Utilization of Small Business Concerns 52.225-13, Restrictions on Certain Foreign Purchases; 52.232.33, Payment by Electronic Funds Transfer Central Contract Registration. A residency requirement certification as stated below, must be completed and returned with the offer/bid/quote for individuals applying for a contract. Companies must submit a completed certification for each of their employees performing work at the Federal Bureau of Prisons, prior to performance. In order to facilitate the clearance process, companies should submit the certifications as soon as it is apparent which employers will be performing the work but no later than one week prior to performance. Documentation to substantiate residency may be requested at anytime prior to award for contracts with individuals, and prior to performance for awards to other than individuals. Department of Justice Residency Requirement Certification: I certify that for three of the five years immediately prior to responding to this solicitation, I have: 1) resided in the United States (U.S.); 2) worked for the U.S. overseas in a Federal or military capacity; or 3) have been dependent of a Federal or military employee serving overseas.
