- This is a combined synopsis/solicitation for commercial items prepared in accordance with the Federal Acquisition Regulation (FAR) sub part 12.6,as supplemental with additional information included in this notice. This announcement constitutes the only solicitation; requests for quotes are being requested and a written solicitation will not be issued. Reference number: RFQ 673-90F-04-0012. This announcement incorporates provisions and clauses in effect through FAR FAC 2001-23. NAICS code is 561621, 500 Employees. This solicitation is 100% set aside for small disabled veteran owned business. James A. Haley VA Hospital has a requirement for an Infant Security System (see attachment "A"). Place of delivery is Offutt AFB, NE68113. Delivery, which shall be FOB Destination, shall occur 30-45 days after award of the purchase order. In accordance with FAR 22.402 (b) the Infant Security System will be subject to the Service Contract Act (FAR 52.222-41) and wage rates and the construction work (Doors, any internal wiring, etc) will be subject to the Davis Bacon Act (FAR 52.222-6) and wage rates. This is an all or nothing request. Offerors must be able to provide the security system, equipment installation, service and minor construction and repair. Construction component is incidental to the security system services i.e., FAR 22.402 (b). For technical questions, please call Mr. Don Schlichting at (402) 294-0121. Offerors responding to this announcement shall provide all information contained in the Federal Acquisition Regulation (FAR) Clause 52.212-1, Instructions to Offeror--Commercial Items. Offerors shall include a completed copy of provision 52.212-3, Offeror Representations and Certification--Commercial Items. An authorized representative of the offeror must sign these representations and certifications; (4) The Offeror shall also provide its Commercial and Government Entity (CAGE) code, Contractor Establishment Code (DUNS number) and Tax Identification number. The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, Contract Terms and Conditions--Commercial Items. Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items, applies to this acquisition.
