- The Federal Bureau of Investigation has a requirement for window cleaning service for the Criminal Justice Information Service (CJIS) located at 1000 Custer Hollow Road, Clarksburg, West Virginia 26306. This requirement is being set aside 100 percent for small business. The CJIS Division has approximately six hundred fifty eight (658) square feet of office space that requires interior and exterior window cleaning. The buildings combined have approximately fifty percent (50%) exterior glass. The heights range from approximately six inches (6?) from the ground to over 63 feet (63?) in total elevation. A site visit will be scheduled for May 21, 2004. The following information must be sent by facsimile to Michael S. Reece at (304)-625-5391 with the following information by May 17, 2004, 1. The period of performance will be for a one-year period from date of award, with four one-year option periods. See note 1. All cleaning service?s shall be conducted at the Facility located at the following address: Federal Bureau of Investigation, Criminal Justice Information Services Division (CJISD), 1000 Custer Hollow Road, Clarksburg, West Virginia 26306, to the attention of the individual who will be identified on the delivery order. Items shall be shipped F.O.B. Destination. (viii) FAR 52.212-1, Instructions to Offerors -- Commercial Items is incorporated by reference and applies to this acquisition. ADDENDUM FAR 52.212-1 Para. (A) The following factors, in descending order of importance, shall be used to evaluate offers: price, and past performance. For past performance evaluation, offerors shall provide names, addresses, and telephone numbers of three customers, Government and commercial, that have utilized the offeror's services for the same or similar item during the past two years.
