Description
This is a nationwide sources sought notice. This is not an invitation for bid, a request for proposals or other solicitation. The Healthier Worksite Initiative and Centers for Disease Control and Prevention (CDC), Office of Occupational Health and Safety, seek to expand nationwide fitness center access for CDC employees, contractors and their family members, to respond to employee requests and promote physical activity by reducing the barriers of distance and cost and increase employee access to fitness centers. The following salient features should be included in the services of any organization being considered for this project: Service Features: 1) Provide access to an already established network of individual and chain fitness centers throughout the United States and San Juan for CDC employees and their families, at a larger membership fee discount than what employees could obtain without this service being offered through CDC. 2) If the provider?s current network does not include fitness centers in some areas where CDC employees work or reside, the provider must be willing to negotiate with local fitness clubs to bring them into the network. 3) Provide flexible memberships that allow employees to use network clubs throughout the U.S. at no extra charge while on travel and permit freezing or transfer of memberships for employees on longer-term assignments away from their local fitness club. 4) Fitness centers included in the network must abide by International Health, Racquet and Sport club Association (IHRSA) membership standards. 5) The provider requires no minimum employee participation. 6) There is no charge to CDC for the above services. Administrative Features: 1) Assign a staff member who is responsible for services provided to CDC employees and communicates regularly with the CDC Healthier Worksite contact person. 2) Provide CDC with aggregate utilization data by geographic area and agreed-to timeframes for program evaluation purposes. Protect employee confidentiality by not identifying individuals in these data sets. 3) Assign a staff member who is responsible for services provided to CDC employees and communicates regularly with the CDC Healthier Worksite contact person. 4) Provide all registration, billing, record keeping, ongoing promotional programs/materials/incentives and employee customer service. 5) Products other than fitness center discounts, such as supplements and diets/counseling, shall not be promoted without prior written approval from CDC. 6) There is no charge to CDC for the above materials, products and services. The North American Industrial Classification System (NAICS) code is 713940 with a size standard of $6.0 million. Interested parties having the capabilities necessary to perform the stated requirements may submit capability statements via email to bbiltz@cdc.gov by May 26, 2004. Capability statements will not be accepted after the due date. Your capability statements should not exceed five (5) pages in length. CAPABILITY STATEMENTS MUST DEMONSTRATE the following: 1) At least 5 years experience and demonstrated success at managing a fitness center discount network, supported by references from large employers. 2) List of the name and address of current network fitness clubs, sorted by each city in which CDC has employees. 3) Examples of promotional materials, such as posters, flyers and hard-copy directories. 4) Capability to show registration, billing, record-keeping, ongoing promotional programs/materials/incentives and employee customer service. 5) Capability to establish (if it is not already established) and maintain a separate URL and customized website for CDC employees that offers registration, a fitness club locator and program information. 6) All capability statements MUST include supporting documentation along with the following: a)company name b)address c)point of contact d)phone/fax/email e)NAICS Codes f)business size and status (e.g., small business, 8(a), veteran-owned small business, service-disabled veteran owned small business, HUBZone small business, small disadvantaged business, and women owned small business); and g)list of clients.
