Description
This is a combined synopsis/solicitation prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a solicitation has been uploaded and will be available on the BOS web page. The following provisions and clauses apply plus any addenda: FAR 52.212-1 Instructions to Offerors - Commercial Items; 52.212-2 Evaluation - Commercial Items; 52.212-3 Offeror Representations and Certifications --Commercial Items; 52.212-4 Contract Terms and Conditions - Commercial Items; and 52.212-5 Contract terms and Conditions to Implement Statutes or Executive Orders - Commercial Items. Electronic solicitations will be available at: http://www.bos.oamm.va.gov/cgi-bin/WebObjects/BosPublic.woa/wa/solicitat on number=244-04-00327 on or about May 10, 2004. NAICS Code is 423450 and the size standard of 500 employees. The proposed procurement is unrestricted. The VA Pittsburgh Healthcare System (VAPHCS), Pittsburgh, PA has a requirement for Disposable Pressure Tranducer Sets on-demand, as-needed basis in accordance with the Specifications. Contractor will provide Item (1) sterile disposable pressure monitoring transducer sets, with an inline blood draw system that provides a closed, needle less, blood sampling system. Transducers must be sterile, individually packaged, pre-assembled, disposable consisting of an in-line blood sampling site, 3-way stopcock, backside test port that is mechanically separated from the fluid path in order to maintain sterility and integrity of the fluid and transducers, 3-cc transducer flush device, macro drip chamber and tubing available in an overall length of 84inches, labels, and built in color coding system. Item (2) provide disposable pressure transducer monitoring set. Transducer set must be sterile, individually packaged, pre-assembled, disposable, consisting of an in-line blood sampling site, 3-way stopcock, backside test port that is mechanically separated from the fluid path in order to maintain the sterility and integrity of the fluid and transducer, 3-cc transducer flush device, macrodrip chamber and tubing available in an overall length of 84 inches, labels, and built in color coding system. Item (3) disposable pressure transducer monitoring set. Set contains a single spike and single drip chamber (macrodrip). The set then trifurcates into three separate transducers. Transducer set must be sterile, individually packaged, pre-assembled, disposable, consisting of an in-line blood sampling site, 3-way stopcock, backside test port that is mechanically separated from the fluid path in order to maintain the sterility and integrity of the fluid and transducer, 3-cc transducer flush device, macrodrip chamber and tubing available in an overall length of 84 inches, labels, and built in color coding system. Item (4) Cardiac Output injectate tubing set. Tubing should function with room-temperature injectate. Kit should contain a length of tubing no less than 72 inches in length. The tubing will have a standard IV bag spike and a pinch clamp on the tubing distal to the spike to prevent the flow of fluid when activated. The tubing will have one-way valve at the other end that allows for connection of an enclosed leur-lock syringe. Port protectors will be in place over all openings. There will be a connection near the one-way valve that will allow for connection of a cardiac output thermistor cable. The enclosed leur-lock syringe will have a permanently affixed sheath over the syringe barrel which will maintain sterility of the fluid in the syringe when used multiple times. The kit will include one additional stopcock and one additional closed port protector. Item (5) provide start-up hardware and supplies to include pressure monitoring cables 15 foot in length, cable must interface with the Hewlett-Packard Merlin monitoring system cardules; transducer holder (pole mount), transducer holder pole clamp, pressure transducer cable tester (must interface with a test port that will not interrupt, contact, or otherwise impede the fluid path, laser levels and cardiac output interface cable (To run from the cardiac output set to the monitor). Item (6) Education materials that will at least include two of the following forms of media: CD/DVD, VHS tape, Booklet, Posters. Proposals must include sterile bid samples (5 each of each line item) and set-up hardware. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (1) Technical capability of the item offered to meet the Government requirement (including submission of descriptive literature); (2) Past performance; (3) Price; all to be submitted with proposal. Solicitation documents have been uploaded with offers due May 28, 2004. Contract effective date estimated to be July 1, 2004 for a period of one year, with 4 option years. Written documents must be submitted; electronic responses will not be accepted. This solicitation is subject to availability of funds.
