Description

Contractor shall provide a Social Worker with at least a Master's Degree in Social work to provide services to the outpatients at the Community Based Outpatient Clinic located in Montevideo, Minnesota. Services to be provided from 8:00 - 5:00 pm, 2 days per week for one month, then two days per month for the remainder of the contract. Contract period will be from date of award for one year with 20 one year option periods. Issue date is 5/7/04. Proposal due date is 6/8/04 at 4:30 pm local time. No hard copies of the solicitation will be available. Faxed or electronic bids will not be accepted. The NAICS code is 621330 with a size standard of $5.0 Million. Contractor must register on www.ccr.gov prior to award.
