Description

Contractor shall provide ambulatory care center, hospital/day surgery center associated with a hospital, or hospital; equipment; supplies and staff necessary to perform, implement and administer outpatient eye surgery for veterans of the VA Outpatient Clinic, FT. Myers, Florida. Contractor facility must be located in the service area of Ft. Myers, Florida (Lee County). Contractor will provide appropriate staff to assist VA physician in preparing patient for surgery, completing operative procedures and recovery from anesthesia. Low Risk background investigations shall be conducted on all Contractor and Subcontractor personnel. The cost of these investigations shall be borne by the Contractor. This solicitation for commercial items will be conducted in accordance with FAR Part 13.5 test program with the procurement conducted in accordance with FAR Part 12 and 15, in accordance with IL 90-99-4. The North American Industry Classification System Code (NAICS) is 621493 and the business size standard is $8.5 Million. Solicitation will be available on or about May 5, 2004, with offers due on or about June 4, 2004. The contract period will be one year from the date of award, with the Government having the option to extend the contract for four (4) additional one-year periods. Contractors may obtain a hard copy of the solicitation and any attachments at the following URL: http://www.bos.oamm.va.gov. Contractors are advises that they are responsible for obtaining any amendments to the solicitation. All offers must be returned to the address in block 9 of Standard Form 1449.
