- THIS IS AN AMENDMENT TO CORRECT ADMINISTRATIVE ERRORS IN SYNOPSIS/SOLICITATION. FIRST PARAGRAPH OF DESCRIPTION IN CORRECTED TO REFLECT SYNOPSIS/SOLICITATION FA4664-04-T-0007 INSTEAD OF F04605-04-T-0007. THIRD PARAGRAPH CONTRACTORS ARE TO FAX REQUESTED INFORMATION FOR BASE ACCESS BY 5 MAY 04 AT 1100. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. The solicitation reference number FA4664-04-T-0007 is being issued as a request for quote. This solicitation document, incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-22. This acquisition is HUBZone set-aside (NOTE 27). The associated NAICS code is 238910 with $12,000,000 size standard. Line Item 0001, FFP Services, Nonpersonal. The contractor shall provide all personnel, equipment, tools, materials, supervision and other items necessary to perform swimming pool demolition in accordance with commercial practices and as identified in the attachment 1, statement of work, dated 26 Apr 2004, at March Air Reserve Base California. Performance Period 150 days. Contractors are to submit the following: A signed and dated quote (to include company name, address, point of contact, phone number, fax number, price and payment terms), a list (including the name, address and phone number and fax number) containing three past performances, and completed provisions FAR 52.212-3, 52.212-3 (Alt 1), 52.219-1, 52.219-1 (Alt 1), DFARS 252.212-7000, and AFRCFAR 5352.228-9100 to the 452 Operational Contracting Office, 1940 Graeber Street (Building 449), Attn Martha ODonnell, March Air Reserve Base CA 92518-1650 by 1500 on 21 May 2004. A mandatory site visit is scheduled for 6 May 2004 at 1000. Contractors are requested to submit on company letterhead the name of the employee(s) who plan to attend the site visit. Submit this information via fax at (909)655-5753 no later than 1100 on 5 May 2004 so that base access can be arranged. To be eligible to receive an award resulting from this solicitation, contractor must be registered in the DoD Central Contractor Registration (CCR) database.
