Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a combined synopsis/solicitation for commercial items prepared in accordance with the Federal Acquisition Regulation (FAR) sub part 12.6, as supplemented with additional informati on included in this notice. This announcement constitutes the only solicitation; proposals are being requested and THERE WILL BE NO WRITTEN REQUESTS FOR QUOTATION ISSUED. The solicitation number for this Request for Quotation is W9114F-04-T-007 . This s olicitation is issued on an unrestricted bases under NAICS number 423450. DESCRIPTION: Eight (8) each PROPAQ ENCORE 206-ELNEO/ADULT base monitor, to monitor ECG, respiration, NIBP, Sp02, two (2) temperature channels, mainstream C02 measuring capabilitie s, two (2) invasive BP channels, including expansion module and built-in printer. Base monitor to include five (5) lead ECG cable (AAMI), 220-240V main supply with continental european main cable, 100-12V main supply with Hospital Grade (US) main cable, N ELLCOR Sp02 finger sensor; and eight (8) each Mainstream C02 Sensor & Airway Adapters.. Offerors responding to this announcement shall provide all information contained in the Federal Acquisition Regulation (FAR) Clause 52.212-1, Instructions to Offeror-C ommercial Items. As stated in this clause, the Government intends to evaluate offers and award a contract without discussions with offerors. Therefore, the offeror’s initial offer should contain the offeror’s best terms from a price and technic al standpoint. However, the Government reserves the right to conduct discussion if later determined by the Contracting Officer to be necessary. Offerors shall include a completed copy of provision 52.212-3, Offeror Representations and Certification— Commerical Items. An authorized representative of the offeror must sign these representations and certifications; The offeror shall also provide its Commercial and Government Entity (CAGE) code, Contractor Establishment Code (DUNS number), and Tax Identif ication number (as required). The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-14, Contract Terms and Conditions—Commercial Items. Clause 52.212-5, Contract terms and Conditions Required to Implement Statutes or Executive Orders—Commercial Items, applies to this acquisition.
