- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a combined synopsis/solicitation for the Kansas Army National Guard Medical Command Unit, RFQ W912JC-04-T-5705. Purpose of this solicitation is to establish an army dental/medical (on-site) clinic. The attached lists the commercial dental items t o be solicited delivered and installed from an established dental equipment dealer/manufacturer. Due to the urgent nature, quotes offering more than 8 weeks delivery & installation will not be considered. You must provide the requested information for the commercial item or descriptive information for each item. The clause at FAR 52.212-4, Contract Terms and Conditions (Apr 1998) and the clause at FAR 52.212-5, Contract terms and conditions Required to Implement Statutes or Executive Order-Commercial Items (Jun 1998) applies to this acquisition. Under paragraph b, the following clauses are incorporated: 52.203-6, Restrictions of Subcontractor Sales to the Government with Alternate I; 52.219-8 Utilization of Small Business Concerns; 52.222-21, Prohibition o f Segregated Facilities; 52.222-26, Equal Opportunity; 52.222-35, Equal Opportunity for Special Disabled Veterans; 52.222-36, Affirmative Action for Workers with Disabilities; 52.222-37, Employment Reports on Special Disabled Veterans; 52.225-13, Restricti on on Certain Foreign Purchases; 52.232-33, Payment by Electronic Funds Transfer â Central Contractor Registration. The clause at DFARS 252.212-7001, Contract Terms and Conditions Required to Implement Statutes to Defense Acquisition of Commercial Ite ms, is applicable to this acquisition. The award will be made to the lowest price, most advantageous to the government, technically acceptable proposal that meets the delivery and installation schedule of no more than 8 weeks.
