Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation document; proposals are being requested and a written solicitation and THERE WILL BE NO WRITTEN REQUEST FOR QUOTATION ISSUED. The solicitation number for this Request for Quotation is 797-FDF2-04-0011. This solicitation document incorporates provisions and clauses in effect through FAC 01-21. The associated NAICS Number is 423430. This solicitation is being issued as an unrestricted requirement for GE Medical Systems equipment as a brand name only. This equipment needs to match exsiting GE Medical Systems equipment at Barksdale AFB. Purchase of GE Medical Systems ID/QA Workstation, #S6571YW. This workstation includes: One Monitor 17_ NEC with Multisync LCD 1850X and eXtra View + (or equivalent). One Computer (CPU) Dell, model number WHL (or equivalent), which includes: One centricity CR SP100 Reader; One QC View Software; One DICOM Store Software; One CD Driver for SMA; One RA600 SMA Module; OneRA 600 Standard with 1 FP; One UPS (110) Volt by Powerware # 5115 (or equivalent); One mouse by Intellimouse 1.3A PS/2 compatible (or equivalent). Accessories to this work station include: Software #K2025KA, which is used to record patient demographic and examination data onto the memory chip that is embedded in the CR X-Ray cassette. GE ID Tablets #K2008KC-Includes: Top shelf stacking cabinet by Cart AFC Industries, Inc (Model: Intellistand 2400-includes casters work surface and 4 horizontal shelves edged with bumper molding, keyboard arms & trays, 4 vertical storage cabinets, CPU holder, Width 24_, Depth 16_, color gray; RISLINK Toolkit Software; (3) RF Read/Write Devise with Cassette Guide; Cables to connect computer to UPS. This system will be used with already in-place equipment. Delivery, which shall occur within 60 calendar days of contract award, shall be FOB DESTINATION to Barksdale Air Force Base. The Contractor is responsible for all freight charges. The provision at 52.212-1, Instruction to Offeror--Commercial Items, applies to this solicitation. The provision at 52.212-2, Evaluation _ Commercial Items, applies to this solicitation. Paragraph (a) of FAR 52.212-2 is added to read: Best Value: Technical capability, past performance, and price. The provision at 52.212-3, Offeror Representations and Certification--Commercial Items, applies to this solicitation. The contractor shall return a completed copy of this provision with its quotation. A copy of the provision may be attained from http://www.arnet.gov/far/. The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, Contract Terms and Conditions--Commercial Items. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items, applies to this acquisition. The Offeror shall also provide its Commercial and Government Entity code, DUNS number, and Tax Identification number. As of October 1, 2003, all Contractors, both large and small, are required to register in the Central Contractor Registration (CCR) database before an award. Contractors are responsible for the accuracy of information added to CCR. To register, go to http://www.ccr.gov/. Offers are due by 4:00pm EST on 21 May 2004 at AFMLO/VASS.
