Description
This is a requirements-type contract to provide ambulatory healthcare services as authorized and within the provision of the Code of Federal Regulations Title 42, Part 136 to eligible Indian Health Service (IHS) Contract Health Service (CHS), beneficiaries residing in the following communities and chapter: Farmington, Aztec, Bloomfield, Flora Vista, LaPlata, New Mexico and Indian beneficiaries receiving treatment at the Four Winds Detoxification Center. The Contractor shall provide all drugs, equipment, facilities, personnel, and supplies to provide the services in accordance with the professional practice, New Mexico State and Federal standards. The entire health care and medical service delivery tasks shall be integrated through the continuous quality improvement activities. These activities shall monitor and evaluate service provesses to improve the quality of patient care and meet JCAHO and/or HCFA standards. Medical services is preferred to be provided by a physician; however if a physician is unavailable the service of mid-level provider is acceptable with physician oversight. Listed services to be provided: General Adult Medical Services; General Pediatric Medical Services; Emergency Medical Care; Diagnostic Radiology Studies; Laboratory Services; Medical Services; and Patient Registration. The Contractor's facility shall be Federally Qualified Health Center (FQHC) designated CMS approval for the term of the contract period. The Contractor facilities must be JCAHO accredited or certified by CMS by the startup of contract, and shall remain in that status for the term of the contract period. The Contractor's professional staff (i.e., physician, physician assistant, nurse, lab technologist) must be board eligible, certified and licensed in their field of specialty and must meet the requirement of the State of New Mexico.
