Description
This is a combined synopsis/solicitation. The GLAC (Great Lakes Acquisition Center) intends to negotiate a firm fixed price contract to provide Board Certified or Board Eligible PSYCHIATRIST SERVICES for the VAMC Tomah, WI. Request for Quotation #RFQ 69D-288-04 entitled "Psychiatrist Services" is hereby issued for commercial items as full and open competition and is prepared in accordance with the format in Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items, Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, Part 13, Simplified Acquisition Procedures, and as supplemented with additional information included in this notice. This announcement incorporates provisions and clauses in effect through FAR FAC 2001-22 and VAAR 97-10. NAICS Code 621112 applies for this procurement and the small business size standard is $8.5 million. This announcement/solicitation constitutes the only bid document that will be issued. A separate written solicitation document will not be issued. Contractor's proposals (cost and technical) are herein requested. A response to requirements listed below shall be submitted on contractor's letterhead or formal quotation form and must address all requirements listed in a separate technical proposal. Contractor's price and technical proposal must contain the name; title and signature of person authorized to submit proposals/bids on behalf of the contractor. The contractor will provide temporary, fill-in services for one (1) Psychiatrist to the VA Medical Center, Tomah, WI, beginning immediately through July 30, 2004 with (1) 2 month option to extend services. Requirements: Must be a citizen of the United States. Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. Current, full and unrestricted license to practice medicine or surgery in a state, Territory, or Commonwealth of the United States, or in the District of Columbia. Board Certified or Board Eligible. Physical Requirements, English Language Proficiency and Basic Life Support Certification are also required. Basic Life Support Certification must be obtained through an organization as recognized by the American Heart Association. Physical Requirements: The contractor must provide documentation that demonstrates that the contractor employee has a current physical that includes a current TB test and Hepatitis B vaccination. Proof that a two-step tuberculosis skin testing is up-to-date must be received prior to starting employment. If the physician has had a negative test within the past 12 months, a single step test will be done and again documentation needs to be provided prior to starting employment. Duties: Perform all duties and responsibilities of a staff psychiatrist. Duties include, but not limited to, admission/discharge, treatment and professional direction of inpatient care (Acute, MH NHCU, and Specialized Units: Posttraumatic Stress Disorder, Substance Abuse, and Dual Diagnosis), outpatient care, psychiatric consultations, rounds, attending treatment teams on Extended Care Inpatient Units, and Compensation and Pension Examinations. The Clinical Director Mental Health Service, or designee, will be responsible for making assignments. Normal work schedule will be to provide eight (8) hours of outpatient care services per day, Monday - Friday from 8:00 am - 4:30 pm CST, excluding federal holidays. with a 1/2 hour lunch break and is not paid. Work hours consist of 40 hours per week to normally include 28 hours per week for outpatient clinic at the VAMC Tomah and 12 hours per week for Consults/Rounds/Team Meeting on Extended Care - Nursing Home Care Units. Psychiatrist will be required to provide evening psychiatric coverage on a scheduled/rotating basis with other psychiatrists on staff. This would be from 4:30 pm each evening to 8:00 am the following day during the business work week. Psychiatrist will be required to provide weekend and holiday psychiatric coverage on a scheduled/rotating basis with other psychiatrists on staff. For weekends - psych weekend call would begin at 4:30 pm on Friday and run through 8:00 am the following Monday.* Note: No premium payment will be made for working federal holidays * A medical officer of the day is on duty 24 hours a day. The psychiatrist scheduled for evening/weekend/holiday call must be available for any psychiatry admission to the Mental Health inpatient units and must also be available by telephone if the Medical Officer of the Day has issues/concerns about psychiatry patients. Mental Health Services include: Acute Psych inpatient unit - 16 beds, Psych-Nursing Home Care inpatient unit (Intermediate type pts) - 17 beds Psych-Nursing Home Care inpatient unit (long-term type pts) - 31 beds Residential Treatment Program - PTSD/Sub Abuse/Dual Diagnosis - 35 beds Compensated Work Therapy/Transitional Residence - 10 beds Outpatient Mental Health services Consults/Rounds/Treatment Teams on Extended Care Service (NHCU) Consults to our Acute Medicine inpatient unit Compensation and Pension Examinations. Must be clinically competent to perform examinations, diagnosis, therapeutic treatment, be effective in handling emergencies, patient management and document in patient records. Must be effective in teaching (patient education and co-workers). Must be effective in planning, and should have sound decision making skills. The physician must have good dictation skills and be able to utilize a centralized dictation system. Training on the dictation system will be provided by the medical center. Individual to provide professional direction and participate in the treatment team (which includes clinical staff - psychologists, nurses, nursing assistants, licensed practical nurses, social workers, social work associates, physician assistants, dietitians, recreation therapists, rehabilitation technicians, addiction therapists), evaluate, develop and implement treatment plans; dictate admission and discharge reports; prepare examining physician's reports for courts on commitment cases, attend court hearings to provide testimony. A progress note must be completed for each episode of outpatient care. An inpatient progress note must be completed in compliance with the Mental Health scope of assessment document. Miscellaneous Duties and Responsibilities: (A) Individual will follow and meet all medical center policies, rules and regulations. (B) The medical center utilizes a computerized medical record system. The physician must be familiar with personal computers, have adequate typing skills to perform data entry (progress notes, physician orders, and electronic encounters), and be able to navigate through windows based computer programs. Training on the computerized system will be provided by the medical center. (C) Individual will participate in organization meetings as requested. (D) The physician is required to be certified in Basic Life Support (BLS) and to maintain the certification during their work at the medical center. (E) Maintains patient confidentiality and security of operations. Work Environment: VAMC Tomah facility has six separate patient care buildings, which are connected with an above ground tramway. Much of the work occurs within a typical office setting, either individually or in meeting with others, or in meeting rooms. Some time will be spent walking to and from the inpatient units on buildings with time spent on inpatient units involved with patients, nursing staff or other staff in activities typical of a mental health care facility. Supervision: Will report to the Clinical Director Mental Health Service/designee. JCAHO: (1) The contractor must perform the required work in accordance with JCAHO standards. Key Personal and Temporary Emergency Substitutions: (1) The contractor shall make NO substitutions of key personal. Qualifications: (A) Personnel assigned by the contractor to perform the services covered by this contract shall be proficient in written and spoken English (38 USC 7402). (B) Contractor's assigned personnel must be computer literate as patient documentation is directly inputted in the patient's electronic medical record. Personnel Policy: The contractor shall be responsible for protecting the personnel furnishing services under this contract. To carry out this responsibility, the contractor shall provide the following for these personnel: - Workers compensation - Professional liability insurance - Health examinations - Income tax withholding, and - Social security payments. The parties agree that such personnel shall not be considered VA employees for any purpose and shall be considered employees of the contractor. Record Keeping: The Tomah VA shall establish and maintain a record keeping system that will record the hours worked by the contractor's employee(s). The payments for any leave including sick leave, holiday or vacation time are the responsibility of the Contractor. Submissions: Provide technical proposal and price proposal. Credential Required: Application package provided by the VA Medical Center Tomah, WI to be completed by the candidate. This includes: Curriculum vitae, current references, signed release of information from candidate, and statement that candidate does not have any physical or mental health condition that would adversely affect his/her ability to carry out assigned duties, and additional credentialing and privileging forms. Candidate will also be required to make application through the VET-PRO Internet process. Candidate will need to be enrolled in VET-PRO by our Credentialing and Privileging Coordinator at the VA Medical Center, Tomah, WI. The website address is: http://fcp.vetpro.org. Contractor's employees must be present at VA and must be actually performing the required services for the period specified in the contract or the contract cost will be decreased accordingly during each billing cycle. A VA record keeping system of contractor hours worked is established through time and attendance logs in the departments being served. The payments to contract personnel for any leave, including sick leave, holiday, or vacation time, is the responsibility of the contractor. Leave granted to contract employees would be in accordance with contractor policies. VA is not obligated to the Contractor during therapist absence. Evaluation of proposals offered will include the following factors: Professional qualifications, clinical skills/interpersonal skills, availability, past performance and price. Award will be made to the offeror with the first acceptable candidate. Evaluation may be made with or without discussions with the offeror(s). All responsible sources that can meet the requirements and provide services as listed above may respond to this solicitation by submitting cost and technical proposals (curriculum vitae) on their company letterhead or bid form. NOTE: The contractor should submit with their offer the following documents: (1) Resumes of the proposed service providers (2) Copy of all current licenses (3) A list of the experience of the proposed service providers (4) Breakdown of Hourly Rate (5) Evidence of Malpractice Insurance (6) Background Information (7) Locum Tenan/short-term clinical needs. (e.g. Past Performance evaluation factors shall include the following submittals: Request contractor to submit the number of times their firm has been cited for sub-standard care; how do they assess patient satisfaction; cite any negative findings over the last 5 years in the National Physician Data base, compliance with VISN 12 credentialing and privileging policies and submit CV's for proposed applicant(s)). Line Item 4: Price per Hour for weeknights if called in _______ Partial Hours will be Pro-rated for Payment Purposes.
