Description
This is a combined synopsis/solicitation for commercial item(s) prepared in accordance with FAR12.6, as supplemented with additional information included in this notice. This announcement constitutes the solicitation. W91YTZ-04-T-0091 is issued as a request for bid. The Solicitation document and incorporated Provisions and Clauses are those in effect through Federal Acquisition Circular (FAC) 2001-22. This Procurement will not be set aside for small business. The SIC Code is 3842; the North American Industry Classification System (NAICS) code is 339112; the FSC 6515. The North Atlantic Regional Contracting Office intends a sole source award to Fresenius USA, Inc. for Walter Reed Army Medical Center, 6900 Georgia Avenue, NW, Washington, DC 20307. Wal ter Reed Army Medical Center requires two (2) Fresenius 2008K Hemodialysis systems, catalog #190305 with bicarb, SMS Option, saline drain buckets, Internal Blood Pressure Module, 10cc Heparin Pump, online clearance, one (1) CRRT Eprom, catalog #190354, ins tallation, and user training. Substitutions are not allowed. Bids will be evaluated in accordance FAR 13.106-2 (b)(3)(ii). Award will be made based on low price received from responsive, responsible bidder. Bids must be for FDA-approved products from vend or who is FDA-approved for the manufacture, distribution, and/or sale of required products. Provide three references of similar Governmental contracts by name, place of business, phone number, and Government contract number. The government reserves the rig ht to make award on the initial bid received without discussions. The Government will consider all bids received by 21 April 2004.
