- Description
- The Montana Army National Guard, through the USPFO for Montana, has a requirement for On-Site Medical Surveillance Testing and Physical Exams. The actual testing and exams will be required at two locations: Combined Support Maintenance Shop, Fort Ha rrison, Helena Montana and at the Army Aviation Support Facility, Helena, Montana. The following types of tests/surveillances are anticipated: Comprehensive Medical and Work History, Physical Exam to include Vital Signs, CBC, UA and Chem 25; Audiometric T esting; Vision Screening; Blood Lead and ZPP Levels; Chest X-Ray (PA) with B Reader; Pulmonary Function Testing; and Prostate Specific Antigen. This acquisition is Set Aside 100% Small Business. A Firm Fixed-Price, Indefinite Delivery/Indefinite Quantity contract is anticipated with a Base Year and up to Four Option Years. All responsible sources may submit a proposal which shall be considered by the agency. Solicitation issuance will be on or about 27 April 2004 and proposals will be due on or about 27 May 2004.
