Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotations are being requested and a written solicitation will not be issued. The solicitation number, N00259-04-T-0019 is issued as a request for quotation (RFQ). The North American Industry Classification System (NAICS) Code for this acquisition is 621111, Size: 8.5. This procurement is a small purchase, unrestricted solicitation. The contractor shall provide CLIN 0001: Non-personal Services for a UROGYNECOLOGIST for the period of April 1, 2004 through 30 September 2004 with an option year award for the period of 01 October 2004 through 30 September 2005 IAW the Statement of Work.
