Description
This is a combined synopsis/solicitation for commercial item(s) prepared in accordance with FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the solicitation. Proposals are requested and a w ritten solicitation will not be issued. W91YTZ-04-T-0077 is issued as the Governments?????? intent to award sole source to Alcon Laboratories for the use of LADARVISION Excimer Laser royalty payments. The solicitation document and incorporated Provisions a nd Clauses are those in effect through Federal Acquisition Circular (FAC) 2001-20. This Procurement is unrestricted. The SIC Code is 3841; the North American Industry Classification System (NAICS) code is 339112. The North Atlantic Regional contracting Off ice intends to issue a firm fixed price requirements contract for the LADARVISION Excimer Laser royalty fees at Walter Reed Army Medical Center (WRAMC), Washington DC 20307. The base period of performance is 1 April 2004 through 31 March 2005 with four opt ions. The following Provisions and Clauses are incorporated and shall remain in full force in any resulting purchase order: FAR 52.212-1 Instructions to Offerors- Commercial Items; FAR 52.212-2 Evaluation of Commercial Items. Technical capability and past performance are approximately equal; FAR 52.212-3 Offeror Representations and certifications; FAR 52.212-4 Contract Terms and Conditions-Commercial Items; FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders; FAR 52.216-21 Requirements; FAR 52.217-8 Option to Extend Services; FAR 52.217-9 Option to Extend the Term of the Contract, the Government may extend the term of the contract by written notice within 60 days provided that the government gives the contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The total duration of this contract will not exceed 60 months. FAR 52.252-1 So licitation Provisions Incorporated By Reference; FAR 52.252-2 Clauses Incorporated by Reference; DFARS 252.204-7004 Required Central Contractor Registration, and DFARS 252.212-7001 Contract Terms and Conditions Required to Implement Statutes or Executive O rders Applicable to Defense Acquisitions of Commercial Items. DFARS 252.201-7000 contracting Officer??????s Representative; Interested parties must provide three references for similar Governmental contracts, including points of contact names, phone numbe rs, places of business, and Government contract numbers. Statement of work may be obtained from the below point of contact. The Government reserves the right to make award on the initial proposals received without discussions.
