Description
This is a combined synopsis/solicitation for commercial items or services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are requested and a separate written solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ), solicitation number FA 7000-04-C-0027, in accordance with FAR Parts 12 and 13. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-15. The North American Classification System (NAICS) number is 51710 and the size standard is 1500 employees. The proposed contract is unrestricted. LINE ITEM 0001 REQUIREMENTS: (1) The requirement is for installation and recurring monthly service of a T-1, point-to-point, data line from the United States Air Force Academy, CO 80840 to Fort Carson, CO 80913. (2) The contractor shall provide a T-1 bandwidth B8Z/ESF formatted circuit, with a vendor provided 19-inch rack mountable, 120 VAC, CSU/DSUs with maintenance and installation, and vendor provided timing traceable to a Stratum-1 source between the United States Air Force Academy, CO 80840 to Fort Carson, CO 80913. (3) The NPA/NNX is 719/333 (4) The terminal equipment for the circuit is CISCO 7500. (5) The Air Force Academy contact points where the router is located are: M/Sgt Patnaude, 719 333-3362 or T/Sgt Gillespie, 719 333-3362 (6) The required interface between the terminal equipment and the vendor provided CSU/DSU is 0/0/0 RS 530. (7) Air Force Academy Facility Information/Data for the new circuit: The building number where the router is located on the Air Force Academy is: Building number 5130 Cathedral Drive, room number: LAN Room, Street Address: Building 5130, Cathedral Drive. (8) The Demark location where the circuit is to be delivered at the Academy is the same location as where the router is located and same as number (7) above. (15) Mailing address for Evans Army Community Hospital, Fort Carson, CO is: Evans Army Community Hospital, Bldg 7500, 7500 Cochrane Circle, Fort Carson, CO 80913. (16) The contractor shall designate a point of contact for issues and questions relating to problems with connectivity, troubleshooting. The designation letter shall be forwarded to the Contracting Specialist and Medical Systems Office, not later than ten calendar days prior to contract start date. (17) The contractor shall be available via telephone for support, 24 hours a day and seven days a week. (18) Contractor personnel authorized to act on behalf of the contractor shall be knowledgeable in the area that they are authorized to act on. If contractor personnel are unable to provide immediate responses and/or corrections to problems, contractor personnel must be able to provide the proper the proper information in a timely manner. (18) The Government shall furnish a point of contact to the selected contractor for this acquisition/contract prior to actual contract start date. (19) The monthly usage of the T-1 line shall not exceed 4999 minutes per month. 1. All responsible sources may submit a quotation, which will be considered by this agency. Facsimile quotations shall not be accepted or considered. This procurement is for the acquisition of a commercial item/service utilizing simplified procedures in accordance with FAR Parts 12 and 13. The F.O.B. for this combined solicitation/acquisition is Destination. Quotations received with other than FOB Destination shall not be considered. b. Quotations shall be provided in writing and be stated as: (1) A one-time installation fee for a T-1 point-to-point data line from the United States Air Force Academy to Fort Carson, CO. (2) The monthly recurring charges for point-to-point data line service for one basic year, plus four optional one-year periods beyond the basic year. The quotation shall be stated as a monthly charge. c. The anticipated effective dates for the basic/initial contract is: 3 May 2004 to 2 May 2005. d. Offeror?s shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications?Commercial Items, with its offer. The provisions at 52.212-2, Evaluation/Commercial Items, applies to this acquisition. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation that will be most advantageous to the Government, price and other factors considered. The Government intends to evaluate quotations and make an award without discussions; however, this does not preclude the Government from conducting discussions if it is in the best interests of the Government to do so. 7.
