Description

This is a solicitation for commercial items prepared in accordance with the subparts in Federal Acquisition Regulation (FAR) 12 and 15 and Veterans Administration Acquisition Regulations (VAAR) 873. A written solicitation will be issued. Solicitation number 673-49-04 applies and is issued as a request for proposals. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-20, The North American Industry Classification Systems (NAICS) code is 621512 size standard is $11.5 million. Provide all supervision, personnel, equipment, material, other items and services necessary to provide bilateral upper or lower extremity venous duplex services on weekends and holidays at the James A. Haley Veterans Hospital, 13000 Bruce B. Downs Blvd., Tampa, FL 33612, in accordance with terms and provisions of solicitation for the period one year from date of award with the option of 4 additional years. Total small business set-aside. All proposals are to be submitted by hard copy to the designated office by COB April 9, 2004. ELECTRONIC VERSIONS OF PROPOSALS ARE NOT ACCEPTABLE. Electronic version and hard copy of solicitation will be available March 29, 2004. Electronic solicitation may be obtained FREE at the following URL: http://www.va.gov/oa&mm/busopp/sols.htm. Any amendments or additional information issued to this solicitation will also be available electronically at the URL address. Offerors are responsible for obtaining any and all amendments or additional information. Requests for hard copy solicitation must be in writing. No telephone requests will be accepted.
