Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; Quotations are being requested and a written solicitation will not be issued. The simplified acquisition procedures of FAR Part 13 will be used in conjunction with the commercial item procedures of FAR Part 12. The solicitation is being issued as a Request for Quotation (RFQ). The solicitation number is RFQ 246-04-01367. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. The NAICS is 561320 and the small business size standard is $11.5M. The procurement is unrestricted. The Hunter Holmes McGuire VA Medical Center, Richmond, VA intends to solicit quotations and award a firm fixed price contract on a competitive basis under the authority of Title 38 USC Section 8153 to the offeror(s) whose quotation meets the requirements of this solicitation. The VA reserves the right to award more than one firm fixed price contract if it is determined to be most advantageous to the government. Period of contract will be established as the April 5, 2004 through September 30, 2004. Quotations are due by Noon, April 1, 2004 local time. The Hunter Holmes McGuire VA Medical Center has an urgent need for the services of one registered diagnostic radiologic technologist (general technologist) to work on-site in Radiology Service. Services will be performed from 6:00 a.m. to 2:30 p.m.; 7:30 a.m. to 4:00 p.m.; 8:00 a.m. to 4:30 p.m. day tour of duty. 4:00 p. m. to 12MN evening tour of duty. 12MN to 8 a.m. night tour of duty, and federal holidays. No payment will be made for scheduled half-hour break. No on call, weekend coverage or after hour services will be performed unless specifically stated in the awarded contract. The Contracting Officer must approve requests for these services in advance and a contract modification must be issued prior to performing work. Any work performed without Contracting Officer approval may not be reimbursable. Contractor shall be responsible for protecting their personnel furnishing services under this contract to include worker?s compensation, professional liability insurance; health examinations; income tax withholding and social security payments. Payment for leave to include, sick, vacation or holiday is the responsibility of the contractor. Personnel of the contractor shall not be considered VA employees for any purpose. Contract general technologist shall: 1) Performs a variety of difficult radiographic examinations. Receives patients, explains method of procedure, positions patients, selects and sets technical factors, sets up and adjusts accessory equipment required, and makes exposures necessary for the requested procedures. Performs procedures such as fluoroscopy, gastrointestinal examination, conventional plain film radiography, tomography, ERCP, myelography, fluoroscopic guided, interventional procedures, portable radiography, digital radiography, and operating room radiography. Sets up the x-ray room. Assures that sterile supplies, contrast materials, catheters, and other required equipment are present and laid out. Prepares and administers contrast material orally, by enema, or, under close control of radiologist, intravenously. Performs examinations of head, trunk and extremities for routine physical examinations and for diagnosis of illness or of injuries of accident victims. Processes exposed radiographs and prepare film-processing chemicals. Maintains records by status tracking in the computer patients examined, examinations performed, views taken, and technical factors used. Provides assistance to personnel of a lesser grade in their duties as required during unusual situations. Keep the rooms neat, clean, and stocked with supplies. Change film badge monthly. Communicate with the customers in a manner, which effectively projects a positive, cooperative image of the organization, and treat our customers with courtesy, dignity and respect. Information and services provided to our customers shall be presented in a thorough and timely manner. Periodic reviews of your methods will be conducted to ensure applicable approaches and circumstances were examined before providing the advice or information. Inform your supervisor or other appropriate officials of any issues or potential incidents where a customer may be dissatisfied or angry as a result of the information given. Performs reception duties by greeting patients, visitors, and employees in person and by telephone, and gathers and relays information. Enters the correct information to register patients upon arrival for diagnostic and interventional radiologic procedures. Determines the need of the patients for escorts. Corrects minor computer equipment failure such as resetting printers, changing ribbons, labels, paper, and making sure that the printers are working properly. Repairs film jackets as necessary to keep jackets neat, clean, and readable. Maintains a checkout system for films on loan. Matches radiographic requests forms with unread films and prepares the studies for reading by the radiologist. Maintains an updated list of patients in the SICU, the CCU, and MICU. Hangs films properly, frequently, and constantly as soon as available, on the respective multiviewers using established guidelines and procedures. Distributes film jackets as often as needed to the different work areas in the Radiology Service using established guidelines. 2) Be certified by the American Registry of Radiologist Technologists (AART). Quotations must include the following for each candidate submitted: 1) hourly rate; 2) resume; 3) copies of current licenses and/or certifications; 4) dates of availability. Offerors may submit one or more candidates. Technical and past performance, when combined, is approximately equal to cost or price. Award will be made to the offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The government intends to evaluate offers and award without discussions. However, the government reserves the right to conduct discussions later if determined necessary by the Contracting Officer.
