- (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) Request for quote (RFQ)DMCUMT04MPG0540. (iii) The RFQ document and incorporated provisions and clauses are those in effect through FAC 2001-21. (iv) The NAICS code is 335929, the size standard is 1000 employees, and the resultant contract will be 100% set-aside for small business. (v) Provide a price for each of the following CLINs. (vi) CLIN 0001AA: Quicktron Cass - 12 scmft-50 "fiber cassette", name brand or equal, quantity 60 each. CLIN 0001AB: Quicktron 812-mm2-150-50 mtp-mtp 150? fiber cable, name brand or equal, quantity 10 each. CLIN 0001AC: Quicktron 812-mm2 - 125-50 mtp-mtp 125? fiber cable, name brand or equal, quantity 12 each. CLIN 0001AD: Quicktron 812-mm2-100 50 mtp-mtp 100? fiber cable, name brand or equal, quantity 12 each. CLIN 0001AE: Quicktron 812-mm2 - 50 - 50 mtp-mtp 50' fiber cable, name brand or equal, quantity 10 each. (vii) Ship to DISA OKC ? Det. Montgomery, 401 East Moore Dr., Bldg. 857, MAFB/Gunter Annex, Montgomery, AL 36114-3001. FOB destination; acceptance at destination. Delivery required no later than 10 business days after date of order; offerors should propose delivery if other than required date. (viii) FAR 52.212-1 applies to this acquisition. (ix) FAR 52.212-2 does not apply to this acquisition. Award will be based on technically acceptable low offer. (x) FAR 52.212-3 applies; vendors are to complete this provision and submit with offer. (xi) FAR 52.212-4 applies to this acquisition.
