- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation document; proposals are being requested and a written solicitation and THERE WILL BE NO WRITTEN REQUEST FOR QUOTATION ISSUED. The solicitation number for this Request for Quotation is 797-FDF2-04-0010. This solicitation document incorporates provisions and clauses in effect through FAC 01-21. This solicitation is 100% set aside for Small Business. The associated NAICS Number is 334516. The Air Force Medical Logistics Office has a requirement (BRAND NAME OR EQUAL) for the following item(s): Quantity of 1 Agilent Technologies Octopole Reaction System (ORS) ICP-MS mainframe with Ni interface, ShieldTorch System and rear duct, Part Number G3272A ? With the following configuration: Comprehensive Spares Kit and Installation; Quantity of 1 , 7500 ICP-MS ChemStation Part Number G3150A - With the following configuration: 1 Year SW Upgrade/Phone Assist; Quantity of 1 ICP-MS Checkout Soln for 7500a and 7500i Sys Install Kit contains: Tuning soln, Dual Mode (1), Dual Mode (2), Wash and Water Blank Solns, Part Number 5184-3565; Quantity of 1 Heat Exchanger, PolyScience Model 3370 Air Cooled Recirculator ? Installation Required Part Number G1879A; Quantity of 1 Autosampler Non-Agilent Prod. Part Number CTAAS500; Quantity of 1 Chemical Analysis Application Consulting ? three-days on-site consulting, training, documentation and implementation services. Requires option Part Number H2149. Agilent Technologies is the manufacture of this equipment. If proposing an EQUAL product, vendor shall include specifications with their quote so it can be evaluated. Delivery, which shall occur within 60 calendar days of contract award, shall be FOB DESTINATION to the Brooks City-Base. The Contractor is responsible for all freight charges. The provision at 52.212-1, Instruction to Offeror--Commercial Items, applies to this solicitation. The provision at 52.212-2, Evaluation ? Commercial Items, applies to this solicitation. Paragraph (a) of FAR 52.212-2 is added to read: Award will be made on best value. The provision at 52.212-3, Offeror Representations and Certification--Commercial Items, applies to this solicitation. The contractor shall return a completed copy of this provision with its quotation. A copy of the provision may be attained from http://www.arnet.gov/far/. The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, Contract Terms and Conditions--Commercial Items. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items, applies to this acquisition. The Offeror shall also provide its Commercial and Government Entity code, DUNS number, and Tax Identification number. As of October 1, 2003, all Contractors, both large and small, are required to register in the Central Contractor Registration (CCR) database before an award. Contractors are responsible for the accuracy of information added to CCR. To register, go to http://www.ccr.gov/. Offers are due by 4:00pm EST on 27 April 2004 at AFMLO/VASS.
