Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation, FA4427-04-Q0005., is issued as a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 01-20 and Defense Acquisition Circular DCN 03-23. If two (2) or more responsive, responsible and technically acceptable small or HUBZone businesses respond, this acquisition will be 100% set-aside for small business, if not, it will be unrestricted. The Federal Supply Class (FSC) is J099. The associated Standard Industrial Classification (SIC) is 7389. The North American Industry Classification System is 235990 and the associated business size standard is $11,500,000. This request for quotation consists of the following: Maintenance of Deltech Compressed Breathing Air Purifier, Pyramid 8000 Series, Model 8DM50 in used by the 60CMS Aircraft Fuel System Section located in Building 14, Travis AFB, California 94535. Item 0001: Quarterly Preventive: (1) Charcoal Vessel (MD#AC12); (2) Pre-Filter Element (MD#8813E) and (3) Labor Cost (one tech 4 hours); Item 0002: Annual Preventive Maintenance: (1) Charcoal Vessel (MD#BA14); (2) After Filer Unit (MD#813E), (3) Intake Filter Replacement and (4) Labor Cost (one tech 4 hours). Contractor servicing will be conducted on a quarterly and annual time frame as indicated. Preventive maintenance is to include removal and replace intake filter on compressor. Check oil levels on compressors and service electric motors. Remove and replace the following filters; Pre-Filter, Carbon Filter, Delmonox Cartridge, and Final Filter. Test/Service/Repair Dewpoint and CO Monitor Alarm Systems to include remote alarm control panel and certify in writing that the unit is ready for use by personnel. A copy of servicing and certification of the unit will be given to the user and a copy forwarded to Bioenvironmental Flight to be kept on file. As authorized to make calls to the contractor for scheduling service will be made by the Equipment Custodians, Mr Zerkel, 707-424-5606 or MSgt Barlak, 707-424-4361. The following provisions and or clauses apply to this acquisition. Offerors must comply with all instruction contained in FAR 52.212-1, Instructions to Offerors-Commercial. FAR 52.212-2 Evaluation-Commercial Items, paragraph (a) to this provision is completed as follows: Evaluation factors shall be price alone. FAR 52.212-3, Offeror Representation and Certification-Commercial Items, a completed copy of this provision shall be submitted with the offer. Quotes without this provision will not be considered for an award.; FAR 52.212-4, Contract Terms and Conditions-Commercial Items; FAR 52.212-5, Contract Terms and Condition Required to Implement Statues or Executive Orders-Commercial Items (Jan 2004) include clauses (b)(4), (5), (8), (13), (15), (16), (17), (18), (19), (29), and (c) (1), and FAR 52.222-42, Employee Class ?
