- Description
- The VA Medical Center, Salem, Virginia has the need for Community-Based Outpatient Center (CBOC) Pharmacy Services for its? eligible beneficiaries. Pharmacy(ies) must be located within a ten (10) mile radius of the Department of Veterans Affairs CBOC?s located at: Tazewell Family Physicians, 123 Ben Bolt Avenue, Tazewell, VA 24651; Southside Family Medical Center, 800 Memorial Drive, Suite D, Danville, VA 24541; Sandy River Medical Center, 4520 Medical Center Road, Axton, VA 24054; Martinsville Family Medical Center, 315 Hospital Drive, Suite 101, Martinsville, VA 24112; and Tri-Area Health Clinic, 14559 Danville Pike, Laurel Fork, VA 24351-3331. It is the VA?s intent to solicit offers and award one or more contracts on a competitive basis. THIS PROCUREMENT IS A 100% SMALL BUSINESS SET ASIDE. Services will be needed for approximately 1600 prescriptions per year. The NAICS code is 446110. Size standard is $6.0M. Contract period will be one year from date of award with four twelve-month renewable option periods. Solicitation packages will be available on or about March 24, 2004. This requirement is subject to the availability of FY04 funds. Solicitation documents may be obtained electronically at: http://www.bos.oamm.va.gov.
