Description

Provide certified ophthalmologist with current licensure to perform ophthalmology services at the Contractor's establishment for beneficiaries of the Veterans Affairs (VA) Medical Center, Augusta, Georgia. Vendors shall be within a 25 mile radius of the VA Medical Center, Augusta, Georgia. Provide routine medical retina examinations to include the following CPT Codes: 92002 - Eye exam new patient - intermediate; 92004 - Eye exam new patient - comprehensive; 92012 - Eye exam established patient - intermediate; 92014 - Eye exam established patient - comprensive. Patients requiring retinal laser surgery or viteroretinal surgery would return to the VA Medical Center, Augusta, Georgia, for definitive care. Contract period is to be May 15, 2004 through May 14, 2005, with four (4) one-year options to extend. This procurement is being conducted undar FAR Part 12 and FAR Part 14. The solicitation is issued as an Invitation for Bids. The NAICS Code is 621320 and the size standard is $6.0 million. The anticipated issue date is April 1, 2004.
