Description
This modification changes the contract instrument to Blanket Purchase Agreement, identifies work locations or zones, performance period, and requests SF254 and SF255?s. The NAICS Code should be: 541370. The Natural Resources Conservation Service (NRCS) is seeking firms to establish either a single or multiple Blanket Purchase Agreements (BPA) for boundary survey and/or topographic surveys for conservation easements in 6 project areas (zones) in Kentucky. The surveys will be used to acquire easements under the Wetlands Reserve Program (WRP), Grassland Reserve Program (GRP), and other Farm Bill related projects in the state of Kentucky. The Blanket Purchase Agreement(s) will be for a three (3) year period beginning on or about May 10, 2004 through May 9, 2007. The projected number of surveys required each year is 0 to 20 in any location. The total expenditure each year of the BPA will not exceed $200,000.00. There is no guaranteed minimum number of surveys to be ordered in any year of the BPA. The work consists of performing land surveys, preparing and furnishing plat maps, electronic (digital) survey files, written descriptions of land surveys, marking of easement boundary and any other items as required by the specifications. The work consists of performing all surveys, measurements, and computations necessary to furnish topographic survey control points, topographic point maps, and contour maps required by the specifications. The services to be provided include all labor, equipment, materials, services and supplies required to complete land surveys, maps, files, reports, and mark boundaries. The work is to be performed by a Registered Land Surveyor of the Commonwealth of Kentucky. The work locations and counties to be covered are identified as follows: Zone 1: Fulton, Hickman, Carlisle, Ballard, McCracken, Graves, Calloway, Marshall, Livingston Zone 2: Trigg, Lyon, Crittenden, Caldwell, Christian, Hopkins, Webster, Union, Henderson, Daviess, McLean, Muhlenberg, Todd, Ohio, Hancock Zone 3: Logan, Butler, Edmonson, Warren, Simpson, Allen, Barren, Hart, Green, Taylor, Adair, Metcalfe, Monroe, Cumberland, Clinton, Russell, Casey, Pulaski, Wayne, McCreary Zone 4: Breckinridge, Grayson, Hardin, Meade, Bullitt, LaRue, Nelson, Marion, Washington, Spencer, Shelby, Oldham, Jefferson, Anderson, Mercer, Boyle, Lincoln, Garrard Zone 5: Timble, Henry, Carroll, Gallatin, Boone, Kenton, Campbell, Owen, Grant, Pendleton, Bracken, Mason, Fleming, Robertson, Nicholas, Bourbon, Harrison, Clark, Madison, Jessamine, Fayette, Woodford, Franklin, Scott Zone 6: Lewis, Greenup, Carter, Rowan, Elliott, Boyd, Lawrence, Bath, Montgomery, Powell, Menifee, Morgan, Martin, Johnson, Magoffin, Wolfe, Estill, Jackson, Rockcastle, Lee, Owsley, Breathitt, Floyd, Pike, Knott, Letcher, Perry, Harlan, Leslie, Clay, Laurel, Whitley, Knox, Bell Firms will be evaluated on the following factors: 1) Licensed Land Surveyor in Kentucky, 2) Experience and recent performance, 3) Equipment submitted for use, 4) Training and Experience of support personnel, 5) Location of permanent employees and their knowledge of the project area. All will be equally weighted in the rating criteria. Interested and qualified firms should submit 2 hard copies to Linda Fryman, USDA-NRCS, 771 Corporate Drive, Suite 210, Lexington, KY 40503, by 4:30 p.m. local time, April 12, 2004 of the following: 1) Standard Form 254 ?Architect-Engineer and Related Questionnaire?, 2) Standard Form 255 ?Architect-Engineer and Related Services Questionnaire for Specific Project?, 3) The project zone or zones in which the firm is interested in performing the work, 4) Any other information pertinent to establishing the BPA.
