Vendor to submit a maximum cost per page to charge requester of informationMILSTRIP: N0018304MDMT141PURCHASE REQUEST NUMBER: N0018304MDMT141 NET AMT FOB: Destination Section C - Descriptions and Specifications CLAUSES INCORPORATED BY FULL TEXT THE MINIMUM REQUIREMENTS OF THIS SOLICIATION ARE AS FOLLOWS: ?h The purpose of this Solicitation is to establish and agreement between Naval Medical Center, Portsmouth, VA and a selected vendor for the purpose of providing coping services of patient records to a third party requestor of records. ?h Vendor shall provide all services required to copy patient records. o Vendors shall supply all labor, equipment, supplies and transportation (including shipping if required) o Vendor shall not remove files from Government property. Vendor will be allowed to scan documents for duplication at off-site facility. o Selected vendor shall establish invoicing agreement with third party requestors of coping services. Vendor must make available (to third party requestors) terms and conditions of payment method prior to delivery of copied material. The Government shall retain no liability in the collection of debits or the payment of services, but the Government reserves the right to audit billing practices. Vendor shall comply with the requirements of DoD Financial Management Regulation (DoD 7000.14-R) (note that this regulation applies restriction on the ability to charge for some duplication services). o Vendor shall not be required to copy files which are prohibited from reimbursement as a result of the DoD Financial Management Regulation (DoD 7000.14-R). Note: DoD Financial Management Regulation (DoD 7000.14-R) prohibits the charging for copies of medical records that are requested by the patient, his or hers next of kin, or legal representative. o There will be no minimum or maximum guarantees of volume as a result of the final awarded contract. ?h Vendor to warrant that all items duplicated have been reproduced in accordance with acceptable industry standards. o Vendor to use 20 lb bond paper or better. Copies are to be legible with no more than 10% distortion from the original document. o Vendor to ensure that all records are safeguarded from unauthorized use or release. Vendor to comply with patient privacy laws including the latest HIPPA requirements. Vendor shall not maintain patient files, within 30 days of duplication all backup files shall be erased / destroyed.
