Description This is a combined synopsis/solicitation for commercial items prepared per the format in Federal Acquisition Regulation (FAR) Subpart 12.6, 13.6, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotations are being requested. No other written solicitation will be issued. The Solicitation number is F0XG2N072001 and is issued as a Request for Quotation (RFQ). This solicitation documents and incorporates provisions and clauses in effect through Federal Acquisition Circular 01-20. This acquisition is unrestricted. The U.S. Air Force 36th Contracting Squadron intends to award a purchase order for the supply and delivery of the following line items: CLIN DESCRIPTION UNIT OF ISSUE QTY UNIT PRICE AMOUNT 0001 Provide 20 minute fireworks show on Andersen AFB, Guam, in accordance with below statement of work. set 1 STATEMENT OF WORK 1. SERVICES NON-PERSONAL: To furnish all labor, equipment, tools, materials, supervision, and transportation necessary to conduct a fully successful fireworks display 4 July 04 on Andersen AFB, Guam. 2. WORK REQUIRED: 2.1. Engineer and conduct a fireworks presentation lasting no less than 20 minutes. 2.1.1. Contractor will provide a detailed list of what?s included in their proposed 20 minute firework presentation. 2.2. 36 Services Squadron will make a site preference to the Contractor not later than 01 June 04. Prior to 01 June 04, contractor will attend meetings with 36 Services Squadron and designated base agencies deemed necessary to coordinate the display. 2.3. Contractor will provide a plan of the fireworks display, as well as a written risk assessment of the selected site to 36 ABW/SE not later than 15 June 04. The Air Force reserves the right to change the location and time of the display if it is in the best interest of safety and the Andersen AFB mission. 2.4. Contractor must comply with safety guidelines in NFPA 1123, Code for Fireworks Display. Display area size will be provided to contractor prior to 01 May 04 or whichever is appropriate prior to assembly and shipment of the fireworks. 2.5. Contractor will be responsible for clean up of fireworks debris. 2.6. Contractor will secure all necessary permits, licenses, necessary to conduct display. Provide all permits and licenses and insurance to 36 Services Squadron/SVF not later than 01 June 04. The Contractor or his representatives will coordinate NOTAM requirements. 3. HOURS OF OPERATION: 3.1 Normal Hours of Operation. The contractor shall perform the services required under this contract during the following hours: Monday ? Friday, 0730 ? 2400 HOURS. All overtime must be coordinated through the 36 Services Squadron (36 SVS) and approved by a Contracting Officer. 3.2 Holidays. The contractor is required to provide service on 04 July 04. Rain date is 05 July 04 4. PLACE OF PERFORMANCE: Andersen AFB, Guam, Palm Tree Golf Course Driving Range. (Final time and location to be determined by 36 Services Squadron). 5. SAFETY 5.1. The contractor shall comply with Air Force Occupational Safety and Health (AFOSH) and Occupational Safety Agency (OSHA) standards and other related and health requirements for the protection of personnel, facilities and equipment. Contractor must provide a copy of liability insurance to 36 SVS/SVF prior to 01June 04. Liability amount must be, at a minimum, within the Industry Standard and must address the method of resolving claims disputes and processing. 5.2. Contractor shall immediately notify 36 SVS/SVF representative of all employee accidents involving injury or damage to Government facilities, equipment or personnel as well as any privately owned possessions. Contractor representative(s) must be available at the 36 SVS/SVF on 07 & 08 July 03, or as required, to administrate any claims that might arise from the firework works display. 5.3. Prior to, 01 June 03,or start of work, the name of the contact manager (POC) must be forwarded to 36 CONS/LGCB for proper coordination. This is a Brand Name or Equal purchase.
