- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotations are being requested and a written solicitation will not be issued. The solicitation number, N00259-04-T-0021 is issued as a request for quotation (RFQ). The North American Industry Classification System (NAICS) Code for this acquisition is 423450, Size: 5.0. This procurement is a small purchase, unrestricted solicitation. The contractor shall provide CLIN 0001, 360 Humprey Lens Analyzer, quan: 1 ea. Clin 0002, 599 Humphrey Auto Refractor Keratometer, quan: 1 ea. Clin 0003, HT2 Humprey Power Table, wheel chair compatible, vertical raise of 14inches from low of 26 inches to high of 40 inches quan: 1 ea. Clin 0004, 995 Corneal Topographer, quan: 1 ea. Clin 0005, Advanced Refractive Diagnostics Module/Humprey software, quan: 1 ea. Clin 00006, Healing Trend/Stars Display Humprey software, quan: 1 ea. Clin 0007, Master Fit Contact Lens Module /Humprey , quan: 1 ea. Clin 0008, Pathfinder Corneal Analysis/Humprey software, quan: 1 ea.
