Description
This is a combined synopsis/solicitation for commercial, non-personal services prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. A quote is being requested and a written solicitation will not be issued. Solicitation number F63CES40680100 is hereby issued as a FOB Destination Request for Quote. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-20. This acquisition is restricted 100% to small business in accordance with FAR 19.5 and is under North American Industry Classification System code 561990, with a small business size standard of $6M. Statement of Objectives Asbestos Labeling Altus AFB, Oklahoma 1. Background Past building inspections have identified asbestos-containing materials (ACM) in many facilities at Altus AFB (AAFB). These inspections were conducted utilizing the Air Force Guidance for Rating and Assessing Damage and Exposure (GRADE) System, USAFOEHL Report 89-072EH0021HGA. Current records indicate approximately 47 facilities at AAFB have mechanical rooms containing ACM. Many of the GRADE 1 or 2 materials are known to be located in crawlspaces, attics, or other confined areas. Re-assessment of the condition of these materials was performed in 2003. Information developed during the 1995 inspections has been input into an in-house database system known as Personal Computer View 3 (PCV3) for managing both asbestos and lead-based paint (LBP). Altus AAFB has converted this database to an ACCESS based database. The 2003 reassessment is contained in a separate report maintained by 97 CES/CEV. 2. Objectives The objective for this project is to label all ACM located in the mechanical rooms in accordance with the requirements of 29 CFR 1910.1001 (j) (4).2.1 Labeling of ACM in the Mechanical Rooms The Base will provide a list of targeted facilities requiring labeling. The Base will provide the contractor with the latest asbestos data on all targeted facilities. This data will be in either electronic form data or hard copy form dependent on the needs of the contractor. Contractor will develop plan for labeling all identified ACM within the mechanical. This plan will be submitted to the base for review to ensure access to all facilities can be coordinated in a timely manner. Contractor will provide requested information to AAFB on all visiting contractors, prior to arrival, to allow for security clearances to be conducted. Contractor will perform the requested labeling of those materials agreed on by both the contractor and the base. 2.3 Reports N/A 3. Deliverables N/A 4. Meetings The Contractor will attend one (1) Project Initiation Meeting in accordance with an approved schedule. 5. Government Furnished Materials Altus AFB will provide an electronic copy of the database information including hardcopies of database if requested. Altus AFB will provide information on the known current status of asbestos material not in the existing database. Altus AFB will provide a list of the approximately 47 facilities containing ACM within the mechanical rooms. Altus AFB will assign a project liaison to assist the Contractor in gaining access to restricted, locked, or otherwise inaccessible areas within each facility where re-assessment will be conducted. 6. Period of Performance The period of Performance for this contract will be 60 calendar days from receipt of Notice to Proceed. 7. The government reserves the right to make award on the initial quote without discussions. Each offeror shall include a completed copy of the provisions at FAR clause 52.212-3, Offeror Representations and Certifications?Commercial Items (Jun 2003) with the quotation. If a copy of the provisions are needed, please request one from the point of contact, and a copy will be faxed. FAR clause 52.212-4 (Oct 2003), Contract Terms and Conditions?Commercial Items, is incorporated by reference and applies to this acquisition with the following addendum: DFARS 252.204-7004 Required Central Contractor Registration (Nov 2003). The clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders?Commercial Items (Oct 2003) and its deviations (June 2003) applies to this acquisition and is incorporated by reference.
