Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; a quotation is being requested and a written solicitation will not be issued. This request for quotation (RFQ), number 04DEH060024, is for the purchase and delivery of the following two (2) items: 10 each portable fume hood (PFH) 24" and 25 each portable fume hood-50. Each PFH-24 and PFH-50 shall include 1 blower, 1 enclosure with hinged cover, 1 HEPA Filter, and 1 (2 pak) Carbon Filter. Due to the nature of the procedures and the space allotted the following specific requirements must also be met: (1) the hood must be portable because of limited space inside a seizure vault and cannot be attached to fix ductwork or external fan systems and be UL approved; (2) Hoods must be relatively small to fit in existing workspaces and must fit on a tabletop workbench with a depth of 18 - 28 inches; (3) No attached work surface - users will be required, at times to lift the hood and place it over container(s) before examining contents; (4) Face velocity to capture errant particles within 50-100 linear feet per minute; and (5) The hood must be capable of removing organic chemical and particulate materials in combination. These are critical elements and hoods not meeting all the requirements cannot be used. The Department of Health and Human Services (DHHS), Program Support Center knows of only one source, Hazard Technology, 406 Headquarters Drive # 5, Millersville, MD 21108. Solicitation documents, provisions and clauses herein are those in effect through Federal Acquisition Circular 97-18. The provision at FAR 52.212-1, Instructions to Offerors--Commercial Items (OCT 2000), and the clause at FAR 52.212-4, Contract Terms and Conditions--Commercial Items (FEB 2002), applies to this acquisition. Offerors shall submit a fully executed copy of the provision at FAR 52.212-3, Offerors Representations and Certifications--Commercial Items (JULY 2002) with their offer. The clause at FAR 52.212-5, Contract Terms and Conditions Require to Implement Statutes or Executive Orders--Commercial Items (APR 2003), applies to this acquisition as well as the following clauses cited therein: FAR 52.222-26, Equal Opportunity (E.O. 11246); FAR 52.222-35, Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era (29 U.S.C. 793); FAR 52-222-36, Affirmative Action for Workers with Disabilities (29 U.S.C. 793); FAR 52.222-37, Employment Reports on Disabled Veterans of the Vietnam Era (38 U.S.C. 4212); FAR 52.225-3, Buy American Act - Supplies (41.U.S.C.) and FAR 52.232-33, Payment by Electronic Funds Transfer-- Central Contractors Registration (31 U.S.C. 3332). This notice is not a request for competitive proposals. Firms having the ability to provide the service, as stated above, shall provide clear and comprehensive information supporting its claim, inclusive of references listing contact person(s) and phone number(s); technical literature; GSA schedule, if applicable; and pricing. A determination by the Government not to compete with this proposed contact based upon responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement.
