- This is a combined synopsis and solicitation for the following commercial items, Item Description: 0001) 1 each of Forklift: diesel, 5-ton, forks raise 115?in, length of forks 72?, lowest entrance height 102x118, sideshifter, sideshifter fork positioner, strobe light, back-up alarm; suggested manufacture: Toyota model no: 5FD50 or equal. This synopsis and solicitation is prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quote (RFQ). Submit written offers (oral offers will not be accepted), on RFQ W812CC-4034-1101. All firms or individuals responding must be registered with the Central Contractor Registration (CCR). This procurement is being issued as 100% SMALL BUSINESS SET-ASIDE. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-16 and Defense Acquisition Circular 91-13. North American Industrial Classification Standard 333924 , Size Standard, 750 employees, applies to this procurement. FOB: Destination for delivery to Unit 35152 Torii Station, Okinawa, Japan 96376-5115. The following provisions and clause apply to this acquisition: FAR 52-211-6 Brand Name or Equal. The provision at FAR 52.212-1, Instructions to Offerors--Commercial Items, applies to this solicitation. Quotes will be evaluated on price related factors only. The provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items, applies to this solicitation and the offeror must include a completed copy of this provision with their proposal. The clause at FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders--Commercial Items, applies to this solicitation.
