Description

The Department of Veteran Affairs Network Business Center in Long Beach, CA intends to issue a Request for Proposal (RFP) on or about March 29, 2004. The requirement of this RFP is to establish a contract to provide all labor, materials, transportation, equipment and supervision, in compliance with federal, state and local regulations, to provide Custom Sterile Surgical Packs for fifteen (15) various Government Healthcare facilities located within VA VISN 18, VISN 19, VISN 20, VISN 21, VISN 22, VISN 16 and DoD Facilities in the same geographical boundaries. There shall be the possibility of adding and/or deleting facilities within these VA geographical parameters at later dates throughout the duration of the contract. The contract itself shall consist of one (1) base year and four (4) optional one-year periods. Vendors will have an approximate forty-five (45) day response period time frame from time of availability. Qualified businesses requesting this RFP must submit their request in writing with complete company name, address, telephone number, and point of contact to: Larry R.
