- Description: This is a combined synopsis/solicitation. The VA Network Business Center, Long Beach, California intends to negotiate a Firm-Fixed price, sole source contract with Osteomed Corporation. The Request for Quote # RFQ 600-598-04sc entitled ?Mandibular Reconstruction Sets with self-drilling screws? is hereby issued for commercial items and is prepared in accordance with the format Subpart 12.6 as supplemented by additional information included in this notice and incorporates provisions and clauses in effect through FAC-20, 2/23/04. The SIC code is 339112 and the size standards in number of employees is 500. This announcement constitutes the only solicitation issued. A separate written solicitation document will not be issued. Osteomed Corporation shall provide the Mandibular Reconstruction set to include the following : 1- set, Mandible Fracture Reconstruction System ? 3.0/4.0 Cannulated Podiatry Fixation System, 2.0/2.4 Cannulated Podiatry Fixation System, with Self-Drilling screws. The Surgical Dept. shall use these sets while performing Jaw, Foot and Ankle Surgery, these sets can also be used to treat ablative cancer procedures, trauma, reconstruction, compression plates, tension plates, orthognathic procedures and true lag screws. Osteomed Corporation is the only known company that provides the unique advanced technology that allows for the use of Mandibular fixation and reconstruction with Self-Drilling screws. The warranty for this equipment shall be for one year. The Delivery Schedule is 30 days after receipt of order. Delivery terms shall be FOB Destination. The product shall be delivered to the VA Greater Los Angeles Healthcare System Supply Warehouse, Los Angeles, California. The Government intends to award contract on a sole source basis to Osteomed Corp. Osteomed shall submit a complete copy of provision FAR 52.212-3, Offeror Representation and Certification- Commercial Items. The following Federal Acquisition Regulation (FAR) Clauses and Provisions also apply to the acquisition: 52.212-1, Instruction to Offerors ? Commercial Items; Notification : Federal Acquisition Regulations require all contractors conducting business with the Government to be registered in two separate and individual online databases: 1) the contractor Performance System (CPS) for online evaluation of your performance; and 2) the Central Contractor Registration (CCR) Government-wide online database for the provision of basic information relative to contract awards [refer to FSAR Clause 52.2121-4 , para (t)}. (a) The offeror represents that, if it is subject to the reporting requirements of 38 U.S.C. 4212(d) (i.e., the VETS-100 report required by FAR clause 52.222-37, Employment Reports on Disabled Veterans and Veterans of the Vietnam Era), it has [ ], has not [ ], submitted the most recent report required by 38 U.S.C. 4212(d). (b) An offeror who checks "has not" may not be awarded a contract until the required report is filed. CONTRACT PAYMENTS (MANDATORY INFORMATION FOR ELECTRONIC FUNDS TRANSFER) Electronic Funds Transfer compliance with 31 CFR 208. To facilitate EFT for contract payments, FAR clause 52.232-34 and 52.232-35 (31 CFR Part 208) requires the submission of information by the contractor to the payment office In addition, incorporated herein by reference are the following VA Acquisition Regulations (VAAR) clauses and provisions: 852.211-71, Guaranty; 852.211-70, Requirements for Operating and Maintenance Manuals.
