Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation is issued as a request for quotation (RFQ) number W91ZLK-04-T-0360. The requirement is for 1 each PAXSCAN 2520 Standard Model 1498-S Gadox screen includes Gadox receptor PN: 11765(r) 24.4 X 19.5 CM; screen, Kodak Lanex scintillator, power supply, command processor w/o ABS (LVDS), cable 5.5 meter interface; cable, DSUB/LEMO; software, viva image viewing and acquisition w/basic imaging workstation; command processor w/ABS, C able, 30.5 meter interface. Provisions applicable to this acquisition are 52.212-1, Instructions to Offerors, Commercial; a completed copy of the provision at 52.212-3, Offeror Representations and Certifications, Commercial Items, must be included with of fers. The clause at 52.212-4, Contract Terms and Conditions, Commercial Items, and 52.212-5, Contract Terms and Conditions Required to implement Statutes or Executive Orders, Commercial Items, applies to this acquisition. Delivery is FOB Destination. Estimated delivery lead time is requested. FAR Part 13.5, p urchase orders using simplified acquisition procedures applies to this acquisition. Evaluation criteria is based on technical capability, price, past performance and timely delivery, technical being more important.
