Description
The purpose of this amendment is to incorporate the statement of work and wage determinations. Drawings may be requested via e-mail at lee.smales@offutt.af.mil. Statement of Work 1.Scope: The work covered by this statement of work consists of installing a new wheelchair lift from an existing concrete stair landing inside Building 500, Offutt AFB, Ne. Reference attached drawing. 2.Description of Work: a. The contractors shall perform all duties to remove existing railings, drywall/plaster soffits, appurtenances above the acoustical ceilings such large air ductworks, electrical conduits and loose wires to accommodate headroom for access to new chair lift. b. The contractor shall rebuild or reroute the items in line item a. to meet all building codes, AF standards, Security requirements of Building 500 and Engineering standards. c. The contractor shall install the smallest lift feasible in this area with collapsible ramp. Other requirements to be included with the lift are as follows: Pit will be required to eliminate a steep ramp off of the lift, Call switches, Phone Jack, Emergency alarm switch, Grab rail. d. New railing should match existing and the entire railing both sides of the stair shall be repainted. e. New ceiling soffit will need to be installed of acoustical tile to match existing ATC. Supporting the existing soffit after notching out for new ceiling may be necessary f. General Contractor must coordinate all work involving each construction discipline. 3. Implementation: a. The contractor will be required to allow personnel access to the entrance door during Monday through Friday 0700-1700. He can work there during daytime hours but has to be considerate of people working in the area. The area is a 24/7 work facility so the contractor may be able to make arrangements with the facility manager to work other hours. Consequently, access to the space will have to be accommodated for during all hours. b. There appears not to be any environmental issues or fire detection systems or sprinkler heads in the vicinity of the soffit/ceiling construction. c. There will be a site visit prior to construction. d. There will be prefinal and final inspection by the Government’s inspector prior to completion. Contractor is not responsible for moving furniture or items placed along the wall. e. Contractor shall perform a daily clean-up. No radios, cell phones, pagers, and recording equipment will be allowed. f. Contractor will have to comply with all security investigations and eligible for security clearances to work within Offutt AFB and Building 500. 4. Wage Determination 94-2325 Rev 25 applies and can be obtained at http://www.wdol.gov/
