- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; a quotation is being requested and a written solicitation will not be issued. Reference number F1SVMP40440700 is hereby issued as a Request for Quotation. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-15. This solicitation is unrestricted and listed under North American Industry Classification System Code 423910, with a small business size standard of 500 employees. The corresponding Standard Industrial Classification code is 2522. Hurlburt Field intends to purchase the following: Lockers and benches: Single-tier ventilated lockers 15” wide x 18” deep x 72” height. Total lockers approximately 179 ea (based on dimensions, numbers could vary by 1) in accordance with attached Statement of Work (SOW) and three diagrams. All transportation and insurance costs are required to be included in the quote. Order shall be FOB Destination. GSA items shall be noted as well as the GSA contract number. If quoting “or equal” items, submit sufficient literature for Subject Matter Experts to perform an effective evaluation. Required delivery shall begin on or before 22 Jun 04 through 09 July 04. Place of delivery shall be 16 SERVICES SQUADRON, ATTN MR. ED VIETH, 150 LOOP ROAD, BLDG. 91007, HURLBURT FIELD FL 32544. The following clauses and provisions are hereby incorporated either by reference or full text and are to remain in full force and effect in any resultant purchase order: FAR 52.212-1, Instructions to Offerors - Commercial Items (OCT 2000). It applies to this acquisition and is amended to read: FAR clauses and provisions can be located via the Internet at: http://farsite.hill.af.mil. Offers shall be evaluated in accordance with FAR 52.212-2, Evaluation of Commercial Items (Jan 1999), which is incorporated into this Request for Quote, with addendum to paragraph (a) as follows: The following factors shall be used to evaluate offers: (i) technical capability of the item(s) offered to meet the Government requirement as stated in this synopsis; (ii) price. The Government will make award to the responsible offeror whose offer conforms to the solicitation and is the greatest value to the Government. Technical capability and price are of equal importance. Each offeror shall include a completed copy of the provisions at FAR 52.212-3, Offeror Representations and Certifications – Commercial Items (July 2002), with the quotation. The clause at FAR 52.212-4, Contract Terms and Conditions – Commercial Items (Feb 2002), applies to this acquisition with the following addendum: DFARS 252.204-7004, Required Central Contractor Registration (Nov 2001). The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders (Apr 2003), applies to this acquisition and is incorporated by reference. The clause at FAR 52.232-33, Payment by Electronic Funds Transfer-Central Contractor Registration (31 U.S.C. 3332) (May 1999) applies to this acquisition and is incorporated by reference. The clause at DFARS 252.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisition of Commercial Items (JUN 2003) applies to this acquisition and is incorporated by reference. A Firm-Fixed-Price Purchase Order will be issued in writing. Submit signed and dated quotations on company letterhead or quotation form. The Government reserves the right to evaluate proposals and award a contract without discussions with offerors. All questions must be in writing and may be faxed to (850) 884-5372 or e-mailed to william.johnson4@hurlburt.af.mil Statement of Work: 16 SVS New Fitness Center Locker/Bench Request LOCKER GENERAL DESCRIPTION Single-tier ventilated lockers 15” wide x 18” deep x 72” height. To include a hat shelf and coat rod with 3 single-prong hooks on sides and back. Locker fillers are necessary for open space to ensure esthetic appearance. Total lockers approximately 179 ea (based on dimensions, numbers could vary by 1 ) Ladies Lockers approximately 76 ea Ladies benches (2) 3ft (1) 5ft (1) 6ft (1) 8ft Men’s Lockers approximately 102 ea Men’s Benches (3) 9ft (1) 7ft LOCKER REQUIREMENTS 00.01 DOOR FRAME All door frame members to be not less then 16 gage formed to a channel shape. Cross frame members of 16 gage channel shapes shall be securely welded to vertical framing members to ensure rigidity. 00.02 BODY Bolt spacing in locker body construction not to exceed 9” o.c. All locker body components shall be made of cold rolled steel. Backs shall be 18 gage; all other body parts shall be 16 gage. 00.03 DOORS Doors shall be formed from one piece 14 gage steel and have channel formations on lock side and hinge side and have right angle flanges on the top and bottom. 00.04 DOORS HANDLE & LATCHING 1, 2 & 3 TIER: Handles shall be recessed in the door and be finger lift control. The 20 gage pocket shall be sufficient depth to prevent a combination padlock, and key padlock from protruding beyond the face of the door. Handle finger lift shall have a padlock eye for use with a 9/32” diameter padlock shackle. It shall have a sound deadening molded comfortable finger lift. Latch hooks on diamond-perforated lockers shall have tamper guards. The latch channel assembly shall be quieted by the use of nylon glides to reduce noise. 00.05 VENTILATION All locker sides and doors shall be perforated with diamond-shaped openings ¾”wide x 1-1/2” high to ensure maximum ventilation and maintain structural strength. 00.06 NUMBER PLATES Each locker shall be supplied with a polished aluminum number plate with black numerals beginning with number 1 to 180. Number plates shall be attached to the face of the door with two aluminum rivets. 00.07 INTERIOR EQUIPMENT Lockers shall have a hat shelf located approximately 9” below the top of the locker. All lockers shall include three single-prong hooks and a coat rod that extends the width of the locker. BENCH REQUIREMENTS 00.08 BENCH TOPS Made from hardwood and finished with clear lacquer 9-1/2”deep x 1-1/4 thick 00.09 BENCH PEDESTALS 16-1/4” high and 14” wide base made of stainless steel which allows for moveable free standing use.
